Isolation of a herpes simplex virus type 1-like agent from the brain of a mountain agile gibbon (Hylobates agilis agilis) with encephalitis.

A captive-born 2-yr-old male mountain agile gibbon (Hylobates agilis agilis) that died of encephalitis harbored a herpes simplex virus type 1 (HSV1)-like agent in the brain. A complete necropsy revealed intensive meningeal congestion with nonsuppurative encephalitis. The virus was recovered from frozen brain tissue in Vero cells.

Early presentation of carcinoma of the small bowel in Crohn's disease ("Crohn's carcinoma"). Case reports and review of the literature.

We report here 3 cases of adenocarcinoma of the small bowel complicating Crohn's disease ("Crohn's carcinoma"). In 2 cases, the adenocarcinoma was discovered in the first attack of enteritis. In only one other case in the literature, a similar presentation was described.

Massive lower gastrointestinal bleeding: an unusual complication of Crohn's disease.

Massive rectal bleeding may complicate Crohn's disease at any stage and tends to be persistent. Limited experience gained with this complication supports the use of selective agiography followed by conservative resection as the management of choice. Results of experimental studies suggest that selective infusion of vasopressors may be safely attempted when hemorrhage complicates Crohn's disease in its early stages but is probably too hazardous in cases in which the disease is more advanced.

External biliary fistula.

External biliary fistulas, once common, are now rare: before the present report of 4 cases only 27 cases have been reported in the English literature since 1900. Review of the records of four patients with external biliary fistula confirmed its occurrence in patients over 50 years of age and the variable site for operning of the fistulous tract. Cholecystectomy provided successful treatment in three of the four patients but the fourth was too ill to undergo an operation; in general, definitive treatment is cholecystectomy, together with excision of the fistulous tract if this takes a direct path through the abdominal wall from the gallbladder, or curettage if the course is devious.
