Background: Several endometriosis classification systems have been proposed and published but the search for a universal language that communicates the complexity, laterality and severity of this disease continues. The authors introduce the Visual-Numeric Endometriosis Scoring System. VNESS is a novel system for describing surgical findings in each compartment of the pelvis in a way that is simple to use, visually intuitive and mirrors a laparoscopic image of the pelvis.

Study Objective: To present and evaluate the feasibility of a new technique of lead-electrode stimulation to the genital nerves using a 2-step, double-passage retropubic/retrograde approach.
Design: Prospective observational study.
Interventions: The procedure was initiated in the retropubic passage by placing the electrode from below through a paravulvar/testicular small incision toward the urogenital diaphragm, guided through the retropubic space along the backside of the pubic bone.

Background: The neuroprosthesis laparoscopic implantation technique for electric pelvic nerve stimulation was introduced to gynaecology over 15 years ago to treat intractable pelvic neuropathic pain. Following this first indication, other applications were developed, particularly in parapleology. The LION procedure developed to assist patients with paraplegia and common problems associated with inertia when confined to a wheelchair could find revolutionary applications in aging medicine and prevention.

How can endometriosis develop within the sciatic nerve; a structure which has nothing in common with the uterus either anatomically or functionally, and why it occurs in the absence of any retroperitoneal/parametric endometriosis, is unknown. A better understanding of the pathophysiology of this enigmatic disease may improve its diagnosis and therapy.
