A 67-year-old man undergoing coronary artery bypass grafting had aortic calcification that prohibited aortic cross-clamping. When ventricular fibrillation developed during surgery, we instituted hypothermic fibrillatory arrest to avoid aortic cross-clamping. In addition to our patient's case, we discuss the advantages and disadvantages of using hypothermic fibrillatory arrest during cardiac surgery.

The most important considerations for interventions are the safety, short- and long-term durability of the repair, and the surgical team's skills and experience. We reviewed the results of our operative series on primary and redo carotid endarterectomy (CEA) data to evaluate the short- and long-term outcomes of surgical intervention.

Examples include performing low anterior resection or colostomy takedown in the setting of prior radiation, severe inflammation, or a narrow pelvis. Circular staplers have made low anastomosis a viable alternative to permanent colostomy in these situations.

Case: An 81-year-old female underwent percutaneous RFA for a solitary intrahepatic metastatic lesion that was her only known site of disease 2 years after definitive surgery and chemotherapy for stage IIIC ovarian adenocarcinoma. She was able to start systemic chemotherapy 1 week after RFA, and continues to be free of disease in the liver.
