During comprehensive treatment using bisphosphonates in cases showing skeletal metastases or those with breakdowns in the skeletal metabolism in the form of osteoporosis, the patients developed complications in the form of osteonecrosis of the jaw. The complication resides in a defect in vascularisation of the skeletal tissue, which is brought about by the properties of nitrogen molecules found in modern bisphosphonates. The ailment in the jaw is provoked by the extraction of a tooth.

The osteonecrosis of jaws is a phenomena newly described in the connection with bisphosphonate therapy. We describe a case of a 41 years old man with multiple myeloma, who developed the osteonecrosis of jaw in the course of antimyeloma therapy. The men had tooth pain from the beginning of the chemotherapy, but the extraction was performed till one month before high dose chemotherapy with peripheral stem cell transplantations, when the conservative therapy was find unsatisfactory.

The authors describe a locally recurrent tumor of the mandible whose microscopic structure and immunohistochemical findings corresponded to an epithelioid hemangioendothelioma. The tumorous cells had an epithelioid character, they created rudimentary vascular lumina, and they focally expressed vascular markers CD 34, CD 31, and F VIII. Locally the tumor grew aggressively.
