Introduction: Brain injury is common following open heart valve surgery. Carbon dioxide insufflation (CDI) has been proposed to reduce the incidence of brain injury by reducing the number of air microemboli entering the bloodstream in surgery. The CO2 Study will evaluate the efficacy and safety of CDI in patients undergoing planned left-sided open heart valve surgery.

Background: AIRWAYS-2 was a large multi-centre cluster randomised controlled trial investigating the effect on functional outcome of a supraglottic airway device (i-gel) versus tracheal intubation (TI) as the initial advanced airway during out-of-hospital cardiac arrest. We aimed to understand why paramedics deviated from their allocated airway management algorithm during AIRWAYS-2.

Methods: This study employed a pragmatic sequential explanatory design utilising retrospective study data collected during the AIRWAYS-2 trial.

Article Synopsis
  • The study aimed to compare the effectiveness of the i-gel supraglottic airway versus tracheal intubation as the first airway management technique for adults experiencing non-traumatic out-of-hospital cardiac arrest.
  • The research was conducted through a randomized controlled trial involving paramedics across four ambulance services in England, with participants automatically enrolled between June 2015 and August 2017.
  • The main outcome measured was the modified Rankin Scale score at hospital discharge or 30 days post-arrest, assessing neurological disability and determining whether the i-gel provided better patient outcomes compared to tracheal intubation.

Background: Recruitment to surgical randomised controlled trials (RCTs) can be challenging. The Sunflower study is a large-scale multi-centre RCT that seeks to establish the clinical and cost effectiveness of pre-operative imaging versus expectant management in patients with symptomatic gallstones undergoing laparoscopic cholecystectomy at low or moderate risk of common bile duct stones. Trials such as Sunflower, with a large recruitment target, rely on teamworking.
