The Immunosuppressive Drugs Scientific Committee of the International Association of Therapeutic Drug Monitoring and Clinical Toxicology established the second consensus report to guide therapeutic drug monitoring (TDM) of everolimus (EVR) and its optimal use in clinical practice 7 years after the first version was published in 2016. This version provides information focused on new developments that have arisen in the last 7 years. For the general aspects of the pharmacology and TDM of EVR that have retained their relevance, readers can refer to the 2016 document.

Article Synopsis
  • There is a pressing need for better treatments to prevent and manage allograft rejection in organ transplantation, as current immunosuppressive therapies come with significant side effects.
  • Extracorporeal photopheresis (ECP) is recommended as an adjunctive therapy for preventing organ rejection, especially in heart and lung transplants, and is being explored for kidney and liver transplants.
  • ECP shows promise as it may provide immunomodulation while reducing the reliance on traditional immunosuppression, particularly benefiting high-risk patients with additional health complications.

Background: In patients with cardiogenic shock the clinical treatment often involves temporary mechanical circulatory support for initial haemodynamic stabilization to enable further assessment of therapeutic strategies. The surgically implanted Impella 5.5 can be used for several indications like ventricular unloading, haemodynamic support during high-risk interventions, and as a bridge-to-transplant strategy.
