This article is a retrospective study comparing the efficacy of Regnauld arthroplasty to first ray osteotomies for the treatment of hallux valgus. One hundred consecutive cases of Regnauld arthroplasties were compared with 100 consecutive first ray osteotomies. One hundred fourty-one patients were available for follow-up, and based on clinical/radiographic examinations, 72 were treated with the osteotomy protocol (group A) and 69 with Regnauld arthroplasty (group B).

The surgical technique and instrumentation of four systems of total knee replacement, two with intramedullary and two with extramedullary alignment systems, are discussed. The differences between the instrumentations, their advantages and disadvantages, and the basic "philosophy" of each technique are compared in four "crucial" operative stages, determinant for the final result: the anteroposterior and distal femoral osteotomies, the tibial osteotomy, and the mechanical axis. In expert hands the two systems are equally reliable.

The authors have studied two cases of progressive diaphyseal dysplasia (Camurati Engelmann's disease). In one case their investigations included bone marrow pressure tests, phlebography and blood gas analysis of the medullary blood. The BMP was raised, and the phlebogram showed venous stasis.
