Background: Surgery used to be the treatment of choice in cases of blunt hepatic injury, but this approach gradually changed over the last two decades as increasing non-operative management (NOM) of splenic injury led to its use for hepatic injury. The improvement in critical care monitoring and computed tomographic scanning, as well as the more frequent use of interventional radiology techniques, has helped to bring about this change to non-operative management. Liver trauma ranges from a small capsular tear, without parenchymal laceration, to massive parenchymal injury with major hepatic vein/retrohepatic vena cava lesions.

Article Synopsis
  • - Trauma significantly increases the risk of venous thromboembolism (VTE), a leading cause of death among young patients, particularly due to complications like deep vein thrombosis (DVT) resulting from extended rest and blood clotting abnormalities.
  • - Thromboprophylaxis—mechanical or pharmacological—could potentially lower mortality and morbidity in trauma patients by reducing VTE risks, but past systematic reviews have shown mixed results and low-quality evidence, prompting the need for updated research.
  • - The objective of new systematic reviews is to evaluate the effects of thromboprophylaxis on mortality and incidence of DVT and pulmonary embolism, while also comparing different intervention methods based on trauma types.
