Some gene polymorphisms have been previously associated individually with tacrolimus efficacy and toxicity, but no long-term study to determine the role of pharmacogene variants in the clinical evolution of liver-transplanted patients has been addressed so far. In the present work, we analyzed the relation between highly-evidenced genetic polymorphisms located in relevant pharmacogenes and the risk of suffering premature death and other comorbidities such as cancer, diabetes mellitus, arterial hypertension, graft rejection, infections and nephrotoxicities in a cohort of 87 patients (8 were excluded due to early loss of follow-up) transplanted at Hospital La Fe in Valencia (Spain) during a 12-year follow-up. Employing a logistic regression model with false discovery rate penalization and Kaplan-Meier analyses, we observed significant association between survival rates and metabolizer genes.

To summarize the long-term complications after pancreas transplantation that affect graft function, a literature search was carried out on the long-term complications of pancreatic transplantation, namely, complications from postoperative 3 mo onwards, in terms of loss of graft function, late infection and vascular complications as pseudoaneurysms. The most relevant reviews and studies were selected to obtain the current evidence on these topics. The definition of graft failure varies among different studies, so it is difficult to evaluate, a standardized definition is of utmost importance to know the magnitude of the problem in all worldwide series.

The portal venous aneurysm(PVA) is a very rare entity, which can be congenital or acquired (if portal hypertension, cirrhosis, pancreatitis, surgical history). Although the discussion is still ongoing, treatment would be indicated in symptomatic PVA, over 30mm or progressive growth. In order to provide an update on the subject due to the low incidence of this finding, we present the case of a 51-year-old woman who comes to the ER for painful obstructive jaundice; the ultrasound shows the presence of cholelitiasis and choledicolitiasis and identifies a nodular image in continuity with the right portal branch of 25x22x21 mm.
