Article Synopsis
  • The study explores how the evolutionary divergence of human leukocyte antigens (HLA) affects transplant outcomes in haploidentical donor transplantations (HIDT), focusing on how this divergence influences the presentation of tumor-associated antigens (TAAs) to T cells.
  • The analysis involved 322 recipient/donor pairs and found that class I HLA genes, particularly HLA-B, significantly contributed to higher divergence rates and were associated with poorer disease-free survival (DFS) and increased relapse rates among mismatched haplotypes.
  • Multivariate analysis highlighted that a high divergence in the HLA-B genes correlates with negative transplant outcomes, indicating the importance of HLA compatibility for successful transplantation.

The development of chronic graft-versus-host disease (GVHD) in 1-year survivors after matched related or unrelated hematopoietic cell transplantation was shown to be associated with higher nonrelapse mortality (NRM) and worse overall survival (OS). The impact of chronic GVHD requiring immunosuppression (IS) for recipients of haploidentical transplantation (HIDT) with post-transplantation cyclophosphamide (PTCy) who have survived to 1 year post-transplantation has not been studied previously and was investigated for this analysis. A total of 322 adult patients who underwent HIDT at our center were included in this study.
