Article Synopsis
  • Weaker immune responses were observed in liver transplant recipients after receiving just two doses of anti-SARS-CoV2 vaccination, prompting a study on the effects of a third dose.
  • In a study involving 122 liver transplant recipients, significant improvements in both humoral (antibodies) and cell-mediated (T-cell activity) immune responses were noted after the third vaccine dose, with 86.4% of previously non-responding patients showing a positive response.
  • Factors such as shorter time since transplantation were linked to reduced immune responses, while the presence of mycophenolate mofetil did not negatively impact the response rates; 60% of patients maintained protective antibodies against the Omicron variant 12 weeks post-third dose

Background: Since the introduction of minimally invasive surgery, new techniques like transabdominal preperitoneal (TAPP) repair have progressively gained acceptance for the treatment of groin hernia. Laparoscopic TAPP (LTAPP) is recommended for bilateral repairs. Likewise, the introduction of robotic platforms has promised additional surgical benefits for robotic TAPP (RTAPP), which are yet to be confirmed.

Anastomotic leakage is the most-feared complication of rectal surgery. Transanal devices have been suggested for anastomotic protection as an alternative to defunctioning stoma, although evidence is conflicting, and no single device is widely used in clinical practice. The aim of this paper is to investigate the safety and efficacy of a transanal tube for the prevention of leakage following laparoscopic rectal cancer resection.

Background: The creation of the pneumoperitoneum is the first step for any minimally invasive surgical procedure. Although rare, iatrogenic vascular or visceral injuries are reported and they are mainly related to the insertion of the first trocar. The Open Veress Assisted (OVA) technique allows a safe maneuver of the first trocar in order to minimize the risk of intraoperative complications during positioning of the first trocar.

Article Synopsis
  • Asymptomatic individuals make up a significant portion (25-45%) of SARS-CoV-2 infections, particularly among those on mild immunosuppressive therapy, leading to potential extended virus spread.
  • A study involving 278 liver transplant recipients in Central Italy found that serology testing every four months effectively increased infection detection rates, revealing that 31 individuals had either past or active SARS-CoV-2 infections.
  • The research indicated that factors like gender and kidney function affected the likelihood of being asymptomatic, and the overall anti-SARS-CoV-2 seroprevalence among liver transplant recipients was similar to the general population in the region (11.2%).

Article Synopsis
  • N-Acylethanolamine acid amidase (NAAA) is crucial for breaking down certain compounds and its inhibition has potential benefits in treating inflammatory diseases and now, colorectal cancer (CRC).
  • In preclinical models, the NAAA inhibitor AM9053 significantly reduced CRC tumor growth and altered the tumor secretome, suggesting that NAAA plays a role in tumor progression through specific molecular pathways.
  • Findings indicate that NAAA could serve as a promising drug target for CRC treatment, as its inhibition impacted cell growth and cycle regulation in cancer cells, with reduced expression noted in human CRC samples.

Indocyanine green (ICG) fluorescence imaging is widely used in abdominal surgery. The implementation of minimally invasive rectal surgery using new methods like robotics or a transanal approach required improvement of optical systems. In that setting, ICG fluorescence optimizes intraoperative vision of anatomical structures by improving blood and lymphatic flow.

Article Synopsis
  • * Results showed that liver transplant recipients had significantly lower immune response rates than healthy individuals for both antibody and immune cell responses.
  • * Factors such as being recently transplanted and taking specific immunosuppressive medications (like mycophenolate mofetil) were linked to this reduced response, although the production of antibodies and cytokines remained closely tied amongst the transplant recipients.

Introduction: Krukenberg tumor (KT) is defined as a secondary neoplasm of the ovary. While ovarian metastases account for about 30% of ovarian tumors, KTs are rare, accounting for about 1-2% of the total. The rarity of KT is at least in part responsible for the lack of a precise clinic-pathological characterization of these tumors.

European Association of the Study of the Liver (EASL) guidelines specify HEV RNA, as well as anti-HEV IgG and IgM as positive markers for acute HEV infection. HEV RNA assay sensitivity limitations may lead to false negative test results in patients with low levels of viremia. Moreover, anti-HEV IgM positivity is not a reliable indicator for distinguishing between acute and resolved infections given the ability of this antibody to persist several months after a resolved infection.
