A patient with severe idiopathic hyperuricemia and hypokalemic alkalosis was followed over a one-year period. A tubulointersitial nephritis consistent with hypokalemic nephropathy was found on biopsy. However, the possibility that the hyperuricemia contributed to the hypokalemia and renal lesion cannot be excluded.

Five studies are reported which were designed to define the effects of an estrogen and a progestogen, alone and in combination, on tissue concentrations (heart, kidney, and liver) of two trace metals (zinc, copper) and four bulk cations (sodium, potassium, calcium, magnesium). Male rats received Premarin (0.08 mg) and/or progesterone (2 mg) in saline intramuscularly 5 days a week for 12 weeks.

After an overnight fast and oral hydration with water, hypertensive subjects developed a significant natriuresis (mean urine sodium excretions increased from 130 to 291 mueq/min. The incidence of a natriuresis (greater than 200 mueq sodium excreted per minute) was 75% in the hypertensive group (16 subjects) compared to 27% in a previously studied normotensive group (22 subjects). The incidence of a carbohydrate-induced antinatriuresis (greater than 30% decrease in urinary sodium excretion) was 62% in the hypertensive group compared to 41% in the normotensive group.

Some unfortunate patients suffer disorders which in one way or another, usually neurologically, severely restrict the larynx in its role as a protector of the lower airway. Aspiration ensues, and unless it can be successfully managed, repeated episodes of a violent pneumonitis may lead to terminal chest problems. In some patients, even the cuffed tracheostomy tubes of new and improved design do not adequately prevent aspiration over an extended time intervel, as evidenced by repeated bouts of aspiration pneumonia despite these cuffed tubes and despite optimal intensive care.
