Introduction: Thyroid nodules are infrequent findings in children, though malignancy rates are higher in this population. The Bethesda System for Reporting Thyroid Cytopathology (TBSRTC) standardizes the reporting of thyroid fine needle aspiration (FNA) specimens and has become a global reference for assessing the risk of malignancy (ROM) of thyroid nodules. The 2023 update includes pediatric-specific risk predictions and management recommendations.

Article Synopsis
  • Pediatric thyroid cancers are rare, making up only 0.7% of tumors, with higher occurrences in women and adolescents; various risk factors include genetic syndromes and exposure to radiation.
  • The study analyzed data from 255 pediatric thyroid cancer cases over 20 years, revealing that secondary tumors were rare (5.1%) but showed higher rates of relapse and surgical complications.
  • Overall survival rates were very high (99% at 1 year, 97% at 10 years), with no significant differences in event-free survival between primary and secondary tumors.

Article Synopsis
  • Appendicitis cases in pediatric patients decreased by 25% during the COVID-19 lockdown compared to the previous year, but the average time to seek help remained consistent.
  • The incidence of peritonitis and complications was similar across both periods, suggesting that the severity of appendicitis did not significantly change despite fewer cases.
  • A shift from laparoscopic to open surgery was observed during the pandemic to minimize the risk of virus transmission, reflecting changes in management strategies rather than delays in diagnosis.

Article Synopsis
  • Splenosis is the abnormal growth of splenic tissue outside of the spleen, typically following trauma or surgery, and it usually appears on highly vascular areas like the omentum or mesentery.
  • This condition is characterized by growth in a sessile (stemless) form, relying on nearby blood vessels for its supply.
  • The case discussed involves a 14-year-old boy with a rare occurrence of intestinal splenosis causing endoluminal growth, leading to a small bowel intussusception that required surgery.

Schwannomas are benign tumors which arise from Schwann cells and take place along peripheral nerves. Extra-temporal facial nerve schwannomas are infrequent and present as painless masses in the parotid region, slow-growing and involvement of the facial nerve. Although rare, they should be taken into account as a differential diagnosis in parotid masses in children.

Hepatic and biliary tract injuries due to traumatic causes are rare in pediatric patients. Of the total number of patients with closed abdominal trauma, less than 9 % have liver injury, and the frequency varies between 2 and 3 % of biliary tract lesions. Currently, the recommended treatment for closed abdominal trauma with liver injury without hemodynamic repercussion is conservative.

Diaphragmatic rupture is a relatively uncommon entity in pediatrics that can occur as a result of a high-impact trauma. Only between 25 and 50% of the cases are detected in the initial evaluation of the patient, which increases the risk of complications. This paper presents the case of an asymptomatic 8-year-old patient who was referred to our institution after a vehicular accident.

Intestinal vascular malformations, especially those in the right colon, are a frequent cause of lower gastrointestinal bleeding in adults, but they are a very rare condition in children. Symptoms include acute hemorrhage, intestinal obstruction, or chronic anemia of uncertain etiology, which is the most frequent form of presentation but the most difficult to diagnose and thus properly treat. We report the case of an 11 year old boy admitted to the Emergency Room with abdominal pain, vomits, hemodynamic decompensation, who required expansion and blood transfusion.
