Article Synopsis
  • Multiple sclerosis (MS) research often overlooks racial and ethnic minorities, particularly Black or African Americans, who are underrepresented in clinical trials despite significant differences in disease outcomes.
  • The EVOLVE-MS-1 study assessed the long-term safety and efficacy of diroximel fumarate in adults with relapsing-remitting MS, focusing on outcomes in Black versus non-Black patients.
  • Of the 1057 participants, only 6.8% were Black, with similar levels of adverse effects reported across both groups, though a higher percentage of Black patients discontinued treatment for reasons such as personal choice or adverse events.

Digital Breast Tomosynthesis (DBT) is a widely used medical imaging modality for breast cancer screening and diagnosis, offering higher spatial resolution and greater detail through its 3D-like breast volume imaging capability. However, the increased data volume also introduces pronounced data imbalance challenges, where only a small fraction of the volume contains suspicious tissue. This further exacerbates the data imbalance due to the case-level distribution in real-world data and leads to learning a trivial classification model that only predicts the majority class.

Background: Primary hypogonadism is a recognised complication in survivors of testicular cancer. However, secondary hypogonadism can result from other causes that suppress the hypothalamic-pituitary axis, including obesity, high dose glucocorticoids, chronic end organ failure, and diabetes. The aim of this study was to explore low total serum testosterone in Australian survivors of testicular cancer and examine associations with body mass index, age, and prior chemotherapy use.

Diroximel fumarate (DRF), ozanimod (OZA) and interferon beta-1a (IFN) are disease-modifying therapies approved for the treatment of relapsing multiple sclerosis. No randomized trials have compared DRF versus OZA and IFN. We compared DRF versus OZA and DRF versus IFN using matching-adjusted indirect comparisons for efficacy outcomes, including annualized relapse rate (ARR), 12- and 24-week confirmed disability progression (CDP) and absence of gadolinium-enhancing (Gd+) T1 lesions and new/newly enlarging T2 lesions.

Article Synopsis
  • Mohs micrographic surgery (MMS) is an effective treatment for melanoma in situ (MIS), but inconsistent surgical techniques make it hard to apply current data broadly.
  • To standardize MMS techniques for MIS in future studies, a modified Delphi method was used, involving expert input through voting on various surgical approaches.
  • The study generated 8 consensus recommendations, with five agreed upon in the first round and the others in the second, aimed at providing uniform guidelines to improve future clinical trials.

Objective: To assess the impact of ejaculatory dysfunction (EjD; failure of emission or retrograde ejaculation) on health-related quality of life (HRQoL) after retroperitoneal lymph node dissection (RPLND) for testicular cancer and explore the efficacy of pseudoephedrine hydrochloride as treatment.

Patients And Methods: In a single arm, phase II trial, patients at ≥6 months after RPLND were invited to complete patient-reported outcome measures (European Organisation for Research and Treatment of Cancer [EORTC] quality of life questionnaire [QLQ]-30-item core, EORTC QLQ-testicular cancer-26, and Brief Male Sexual Function Inventory) evaluating HRQoL and sexual function in follow-up (ACTRN12622000537752/12622000542796). If EjD was reported, post-ejaculatory urine ± semen analysis was undertaken.

Article Synopsis
  • Pregnancy is a key consideration for individuals with multiple sclerosis (pwMS) since MS typically begins during the child-bearing age, affecting Disease-Modifying Therapy (DMT) choice before, during, and after pregnancy.
  • This study analyzed data from the US MarketScan Claims Database, focusing on women aged 18-55 with MS from January 2016 to April 2021, aiming to understand DMT use and relapse patterns throughout the pregnancy and postpartum periods.
  • Results showed that DMT usage decreased significantly during pregnancy, particularly among Medicaid patients, who had lower DMT utilization compared to commercially insured patients, indicating a need for better guidance on MS management during these times.

BACKGROUND Incisional flank hernias represent a complication after lateral lumbar spine surgery. Given the increasing rate of lateral lumbar interbody fusions, the rate of incisional flank hernias will increase. Since there are no reports of open massive flank hernia repair utilizing preoperative botulinum injections, we sought to publish this technique to provide surgeons with an innovative method for preoperatively treating patients with massive flank hernias.

Introduction: In EVOLVE-MS-1 (NCT02634307), mean absolute lymphocyte count (ALC) on diroximel fumarate (DRF) declined from baseline by approximately 28% in year 1, then stabilized, similar to ALC decline observed with dimethyl fumarate (DMF). Prior studies reported that clinical efficacy of DMF was not substantially different in patients with and without lymphopenia.

Methods: EVOLVE-MS-1-an open-label, 96-week, phase 3 study-assessed DRF safety and exploratory efficacy in patients with relapsing-remitting multiple sclerosis.

Introduction: Switching disease-modifying therapy (DMT) may be considered for relapsing-remitting multiple sclerosis (RRMS) if a patient's current therapy is no longer optimal. This was particularly important during the recent COVID-19 pandemic because of considerations around immune deficiency and impaired vaccine response associated with B cell-depleting DMTs. This real-world, single-center study aimed to evaluate change or decline in functional ability and overall disease stability in people with RRMS who were switched from B cell-depleting ocrelizumab (OCRE) to diroximel fumarate (DRF) because of safety concern related to the COVID-19 pandemic.

The central hallmark of Parkinson's disease pathology is the aggregation of the α-synuclein protein, which, in its healthy form, is associated with lipid membranes. Purified monomeric α-synuclein is relatively stable , but its aggregation can be triggered by the presence of lipid vesicles. Despite this central importance of lipids in the context of α-synuclein aggregation, their detailed mechanistic role in this process has not been established to date.
