Background: Schistosomiasis has been noted to be one of the most widespread parasitic diseases in the world. It is commonest in developing countries and 85% of the infections have been reported in Sub-Saharan Africa. Presentation with an appendiceal mass has been a rarity among the cases reported thus far.

A 23 year old man presented with intestinal evisceration from stab injury to the left side of the abdomen with a hand-held cow horn at a local night party. He complained of severe abdominal pain and bleeding at the site of injury. He was hemodynamically stable.
