We introduce the experimental and theoretical investigation of the mutual influence of two four-wave mixing (FWM) processes during the propagation of high-power nanosecond pulses at 1030 nm and 1562 nm wavelengths in the few-mode optical fiber. Both fundamental mode FWM (FM FWM) of laser pulses at these wavelengths, and intermodal FWM (IM FWM) of 1030 nm pulses, have common anti-Stokes components at 1003 nm wavelength. This leads to the mutual influence of FWM effects.

The aim of the study was to investigate the role of early multiple organ failure in 152 patients with severe acute pancreatitis admitted to the intensive care unit for the period from 2002 to 2009. The group of patients with early progressive multiple organ failure had high early (29%) and overall mortality (45%) rate, infectious complications rate (39%) and long intensive care unit stay (median - 8 (5; 18) days). Based on the statistical analysis of data the criteria to predicted risk progression of multiple organ failure on admission were: APACHE II score ≥12, SOFA score ≥4, failure >2 organs/systems, Ranson score ≥7.

We explore for all wavevectors through the Brillouin zone the dynamic spin susceptibility χ(total)(+,-)(ω, q) that takes into account the interplay of localized and itinerant charge carriers. The imaginary part, Imχ(total)(+,-)(ω, q), has peaks at the antiferromagnetic wavevector Q = (π, π) and a diffusive-like, extremely narrow and sharp peak (symmetric ring of maxima |q| = q(0)) at very small wavevectors Q(0) is proportional to w/J ≈ 10(-6) with the nuclear magnetic/quadrupole resonance frequency ω and the superexchange coupling constant J. We demonstrate the capability of Imχ(total)(+,-)(ω, q) for plane copper (63)(1/T(1)) and oxygen (17)(1/T(1)) nuclear spin-lattice relaxation rate calculations from carrier free right up to optimally doped La(2 - x)Sr(x)CuO(4) and obtain the basic features of temperature and doping behavior for (63)(1/T(1)) in agreement with experimental observations.

We present a simple derivation of an expression for the superfluid density n(s) α 1/λ(2) in superconductors with the tight binding energy dispersion. The derived expression is discussed in detail because of its distinction from the known expressions for ordinary superconductors with parabolic energy dispersion. We apply this expression for the experimental data analysis of the isotope effect in London penetration depth parameter λ in the BiSrCuO and YBaCuO family compounds near optimal doping, taking into account the orthorhombic distortion of crystal structure, and estimate the isotopic change of hopping parameters from the experimental data.

In an anesthesiologist's practice, except the occurrence of clinical death on the operating table a long-term complete suppression of the electrical activity of the brain may arise only from the medical staff's crude error when administering intravenous anesthetics from the unlikely breakage of the anesthesia apparatus vaporizer. However, the authors assert that such a phenomenon may develop during general anesthesia using propofol given in its usual doses. Clinical examples are given.

Treatment results of 336 patients in critical state who had required prolonged (3-115 days) artificial pulmonary ventilation were analyzed. 3 groups were defined: 98 patients with complications after operations on thoracic and abdominal organs and organ failure development; 55 patients with surgical infection and sepsis; 183 patients with extensive burning injury III AB-IV degrees. Particular features of prolonged artificial pulmonary ventilation were studied.

The study of opinions of the patients dwelling in the various regions of the Russian Federation concerning their treatment in the curative preventive institutions of Samarskaya Oblast. Among nonresident patients 52% had an opportunity to pay for additional medical services in the medical institutions of Samarskaya Oblast. This fact testify that among population of neighboring regions the solvent demand of certain kinds of medical care is formed.

The data on clofelin effect on the course of total anesthesia are contradictory. Clofelin effect on hemodynamics, drug doses, depth of anesthesia, and time of awakening after surgery was studied in 28 male patients with extensive deep thermal injuries in a state of acute burn toxemia and septicotoxemia. Randomized double blind study was carried out in 2 groups: study group, in which clofelin was injected intravenously (2.

Use of propofol in surgery on critical patients is limited because of the possibility of poorly corrected hypotonia. The effects of total anesthesia with propofol + fentanyl + nitric oxide under conditions of artificial ventilation of the lungs (tracrium) were evaluated during 63 operations on 42 patients with severe burns (ASA III-IV), divided into 2 groups depending on the protocol of anesthesia. The difference in the protocols consisted in the method of choice of anesthetic doses: in group A we proceeded from EEG data (bispectral index--BIS) and in group B relied only on general clinical and hemodynamic signs with a retrospective analysis of BIS.

A new method of electroencephalogram (EEG) processing is discussed: bispectral index (BIS), used for monitoring the depth of total anesthesia. The technology of BIS estimation is a principally novel approach to EEG processing, which can be denoted as an expert conclusion. EEG monitor Aspect A-100 (Aspect Medical System, USA) has been used for 3 years in the operation room during narcoses with barbiturates, propofol, midasolam, fentanyl, calypsol, isoflurane, and nitric oxide.

[Marriage in old age groups (medico-social aspects)].

Probl Sotsialnoi Gig Zdravookhranenniiai Istor Med

October 2001

Recently married elderly people change attitude to hospital treatment and living in asylums; they prefer care rendered at home and hospital-substituting forms of care; hospitalization and ambulance care are less needed and permanent social workers are required less. All this promotes rational utilization of the potentialities of therapeutic institutions and saving of public health and social protection resources.

A study on the testing unit was made of a contractile function of ureteral fragments removed at reconstructive operations for vesicoureteral reflux (VUR), neuromuscular dysplasia (NMD) and pelvoureteral segment stricture (PUSS). The study parameters (wall rigidity, amplitude of the contractions, potassium depolarization contracture, responses to adrenoreactive drugs) in patients with VUR were close to normal, whereas in those with NMD or PUSS changed significantly, especially in PUSS patients. Ureteral contractility impairment was heterogenic in all the groups, varying from intact to lost and did not correlate either with age or the disease stage.
