Spanish consensus on sexual health in men and women over 50.

Sexual health has been defined as "the state of physical, emotional and social wellbeing related to sexuality. However, there are medical, psychological and social reasons that complicate full sexual health that are frequently not attended to sufficiently. The objective of this guide will be to analyze the factors that impact the sexual health of men and women over 50 and to provide recommendations for the most appropriate diagnostic and therapeutic measures for this age group.

[Renal transplantation: ethical issues].

One of the most significant advances in medicine during the last 50 years is the development of organ transplantation. In the context of chronic kidney diseases, renal transplantation offers patients a better clinical outcome than other treatment options. However, the benefits of organ transplantation have not been maximized due to an inadequate supply of organs for transplantation.

Locating the active sites of enzymes using mechanical properties.

We have applied the calculation of mechanical properties to a dataset of almost 100 enzymes to determine the extent to which catalytic residues have distinct properties. Specifically, we have calculated force constants describing the ease of moving any given amino acid residue with respect to the other residues in the protein. The results show that catalytic residues are invariably associated with high force constants.

Desmin-related myopathy: clinical, electrophysiological, radiological, neuropathological and genetic studies.

Ten Spanish patients from six unrelated families diagnosed with desmin-related myopathy (DRM) were studied. The pattern of DRM inheritance was autosomal dominant in three families, autosomal recessive in one, and there was no family history in two cases. The disease onset was in early adulthood.

Naturally occurring, postnatal cell death in the cerebral cortex of the micrencephalic rat induced by prenatal X-irradiation.

Naturally occurring postnatal cell death in the somatosensory cortex and medial cortical areas was examined in micrencephalic rats produced after exposure to 100 cGy X-rays at embryonic day 15. The absolute numbers of dead cells per tissue section were not affected in the future subcortical white matter, but were severely decreased in the cerebral cortex when compared with age-matched controls. Furthermore, the proportion of cortical dead cells per 1000 live neurons in the cerebral cortex and the cumulative cell death across time were significantly reduced in irradiated animals.

Late perforation by pacemaker catheter loop in the neck: a preventable complication of the external jugular approach.

The external jugular vein is a less favored but still useful route for insertion of cardiac pacemaker catheters. When this approach is used, gradual descent of the pulse-generator in the subcutaneous tissue of the chest wall may produce sharp angulation of the catheter as it enters the vein. This may result, many months later, in pressure necrosis of the overlying skin.

Pacemaker runaway following intermittent output failure.

A case with a mixed pattern of pacemaker failure is presented. "Runaway" pacemaker with ventricular capture at 250 beats per minute occurred three days after the development of syncope associated with intermittent loss of pacing artifact. Presumed protection by placement of a temporary pacemaker upon admission was unfounded.

The medical record: cui bono? an appeal for personal custody of health data.

A personal, portable, continuing health record is an important adjunct to superior medical care in a complex and mobile modern society, and the medical profession should encourage and facilitate its voluntary adoption by patients.

Nephrogenic pleural effusion.

Although the presence of a pleural effusion is almost always indicative of an intrathoracic problem reflection will yield a certain number of extrathoracic causes, such as hypoproteinemia, Meigs' syndrome pancreatitis and subphrenic abscess. The kidney is a close neighbor of the diaphragm and pleural cavity, and a case of renal stone associated with a small ipsilateral pleural effusion, which regressed with resolution of the primary process, is reported.

Acute hemorrhagic incarceration of prolapsed gastric mucosa.

In some patients with hiatus hernia, vomiting may propel a plug of redundant gastric mucosa into the distal esophagus, where it becomes incarcerated and engorged. Significant bleeding may result. Reduction of the hernia controls the hemorrhage.
