Introduction: Training novice ophthalmology residents on the EyeSi simulator increases cataract surgery safety. However, there is no consensus regarding how much training residents should perform before their first time on patients. We evaluated the French national training program through the analysis of the learning curves of novice residents.

Introduction: Total nephrectomies for the treatment of Wilms' tumor (WT) are more and more performed by laparoscopy, although indications for this approach following the UMBRELLA guidelines are currently very restrictive. The purpose of this study was to assess the compliance to the criteria of the UMBRELLA protocol for minimally invasive approach of WT.

Methods: This retrospective multicenter study included children operated on by laparoscopic total nephrectomy for suspected WT before 2020.

Introduction: Cataract surgery is the most common surgical procedure performed in France. While the incidence of intraoperative complications affecting visual prognosis is extremely low, given the large number of patients operated on, the absolute number of patients affected by complications is quite high. Complication rates are significantly higher when ophthalmology residents (ORs) perform the surgery.

Objective: Despite recent progress in caring for patients born with esophageal atresia (EA), undernutrition and stunting remain common. Our study objective was to assess nutritional status in the first year after birth with EA and to identify factors associated with growth failure.

Study Design: We conducted a population-based study of all infants born in France with EA between 2010 and 2016.

Article Synopsis
  • Long gap esophageal atresia (EA) presents significant challenges for pediatric surgeons compared to non-long gap EA/tracheo-esophageal fistula (TEF), particularly in terms of patient outcomes at ages 1 and 6.
  • A study analyzed data from patients who underwent EA surgery in France, finding that those with long gap EA experienced more complications, longer hospital stays, and increased reliance on parenteral nutrition during the first year of life.
  • At the 6-year mark, patients with long gap EA had more digestive issues, although they showed less spine deformation, with no major differences in outcomes between initial treatment approaches of delayed primary anastomosis or esophageal replacement.

Article Synopsis
  • - The study assessed long-term functional outcomes of children aged 6-30 who had surgery for anorectal malformations (ARMs) in various expert centers in France.
  • - Findings from 367 patients indicated a significant correlation between the rectal pouch position and issues like voluntary bowel movements and soiling, as well as an increased risk of constipation linked to certain anatomical factors and surgical approaches.
  • - The study emphasizes the importance of ongoing follow-up for these patients to address their specific needs and improve their functional outcomes over time.

This study aimed to create a consensus statement on the indications, applications, and limitations of robotics in pediatric urology. After a panel and interactive discussion focused on pediatric robotics, a televoting with 10 questions was administered to 100 pediatric surgeons/urologists attending the joint meeting of the French Group of Pediatric Laparoscopy (GECI)/Italian Society of Videosurgery in Infancy (SIVI) in 2021. The results of televoting were analyzed electronically using Mentometer software.

Introduction: Gastro-esophageal reflux disease (GERD) is the most frequent long-term morbidity of congenital diaphragmatic hernia (CDH) survivors. Performing a preventive fundoplication during CDH repair remains controversial. This study aimed to: (1) Analyze the variability in practices regarding preventive fundoplication; (2) Identify predictive factors for fundoplication.

Article Synopsis
  • Robotics is rarely used in pediatric oncology due to the rarity of childhood cancers, but a large study shows it can be safe and effective in selected cases.
  • Over 93 procedures on 100 tumors were performed in 89 children, with the most common tumors being neuroblastic and renal; most surgeries had no complications and a short hospital stay.
  • Guidelines for robotic surgery in pediatric oncology should involve certified tumor board discussions to ensure safe patient selection.

Background And Objectives: Respiratory diseases are common in children with esophageal atresia (EA), leading to increased morbidity and mortality in the first year. The primary study objective was to identify the factors associated with readmissions for respiratory causes in the first year in EA children.

Methods: A population-based study.

Intractable ascites is a rare condition in children mainly caused by cirrhosis or lymphatic disorders. Internal drainage may be considered as rescue therapy. In our department, 4 patients ages from 2 months to 15 years old underwent a peritoneovenous shunt (PVS) placement between 2010 and 2020.

Objective: To identify the risk factors for early mortality and morbidity in a population with distal esophageal atresia (EA)-tracheoesophageal fistula.

Study Design: Cohort study from a national register. Main outcomes and measures included early mortality, hospital length of stay (LoS), need for nutritional support at 1 year of age as a proxy measure of morbidity, and complications during the first year of life.

The widespread use of robotic surgery has provided surgeons a high-quality and alternative method to perform pyeloplasty. Indeed, robot-assisted technology has made it a lot easier to obtain the advanced technical skills required to perform this procedure. The learning curve (LC) reflects the increase in learning as a function of experience.

Introduction:  There have been numerous reports of robotic pediatric surgery in the literature, particularly regarding urological procedures for school-aged children. Thoracic procedures appear to be less common, despite the fact that encouraging results were reported more than 10 years. Our aim was to report a national experience of esophageal robotic-assisted thoracoscopic surgery (ERATS) and to discuss the most appropriate indications.

In Hirschsprung's disease (HSCR), postoperative course remains unpredictable. Our aim was to define predictive factors of the main postoperative complications: obstructive symptoms (OS) and Hirschsprung-associated enterocolitis (HAEC). In this prospective multicentre cohort study, samples of resected bowel were collected at time of surgery in 18 neonates with short-segment HSCR in tertiary care hospitals.

Article Synopsis
  • The study aimed to identify factors influencing the need for antireflux surgery in infants with esophageal atresia during their first year of life.
  • From a total of 835 infants in a French registry, 682 were evaluated; 53 (7.8%) underwent surgery, with various predictors identified, such as anastomotic tension and low birth weight.
  • Key findings suggest that complications like gastroesophageal reflux, poor nutrition, and surgical difficulties significantly increase the likelihood of requiring antireflux surgery in these patients.

A 14-year-old boy was admitted to the hospital after an episode of blunt trauma to the thorax, resulting in a Chance fracture of L1 and a compressive chylothorax 72h after admission. After initial drainage in the operating room, conservative management was successful. This case study documents one of the rare complications of spinal fractures in the context of high-energy blunt trauma.

Introduction: Laparoscopic surgery (LS) may lead to musculoskeletal disorders (MSDs) and an increase in physical and mental workloads to the surgeon. Robot-assisted surgery (RAS) should improve the ergonomy of the surgeon. This study assesses the experience influence in surgical ergonomics between LS and RAS.

We described the initial experience of four referral centers in the treatment of primary obstructive megaureter (POM) in children, by high-pressure balloon dilatation (HPBD) of the ureterovesical junction with double JJ stenting. We managed a retrospective multicenter study to assess its effectiveness in long-term. We reviewed the medical records of all children who underwent HPBD for POM that require surgical treatment from May 2012 to December 2017 in four different institutions.

Background: With the rapid adoption of the robotic surgery, more and more learning curve (LC) papers are being published but there is no set definition of what should constitute a rigorous analysis and represent a true LC. A systematic review of the robotic surgical literature was undertaken to determine the range and heterogeneity of parameters reported in studies assessing the LC in robotic surgery.

Methods: The search was conducted in July 2017 in PubMed.

Introduction: Few studies have evaluated the learning curve (LC) for robot-assisted laparoscopic pyeloplasty (RALP) for ureteropelvic junction obstruction in children. It was attempted to assess the LC of this procedure using a multi-outcome approach, accounting for patient complexity.

Material And Methods: Data on the first series of children undergoing RALP between November 2007 and December 2017 at the study institution were prospectively collected.
