Febrile neutropenia (FN) is one of the most important life-threatening complications in haemato-oncology. Our objective was to report all-cause mortality rates in patients ill with a hematological malignancy (HM) hospitalized with a first FN episode and to identify predictors for mortality. Chronic lymphocytic leukemia (CLL), the most common adult's leukemia in the western world, is caused in 95% of the cases by uncontrolled proliferation of monoclonal B-lymphocytes. The complement system in CLL is chronically activated at a low level the classical pathway (CP). This chronic activation is induced by IgG-hexamers, which are formed after binding to alpha-2-macroglobulin (A2M).

The study links two intriguing observations about chronic lymphocytic leukemia and Gilbert's syndrome - the oxidative stress affecting the former, and the anti-oxidative effects of the latter. Our pilot study showed that compared to the general CLL population, the GS/CLL cohort less commonly required therapy and demonstrated a reduced CLL-related mortality. Our findings prompt a speculation that elevated bilirubin in GS could hypothetically attenuate the oxidative stress thereby exerting a safeguarding effect on leukemia pathogenesis.
