In 78 from 121 patients with peptic ulcers the cause of the appearance of the ulcer was the insufficient volume of resection of the stomach. As a rule resection was performed with transformation of gastrojejunal anastomosis into gastroduodenal one with the abdominal bilateral subdiaphragmatic vagotomy. The isolated vagotomy in peptic ulcers after resection of the stomach was not fulfilled as a rule.

In treatment of recurrent and peptic postvagotomy ulcer, the performance of gastric resection is indicated. Of 53 patients with a recurrent ulcer, 20 had a "difficult" low penetrating ulcer. With the aim ot prevent duodenal suture failure, gastric resection in these patients was performed using the developed by the authors method which permitted to ensure safety of the single-layer suture of a duodenal stump with preservation of food passage through the duodenum.

The article gives a review of literature on the determination of surgical tactics in patients with duodenal ulcer complicated by bleeding. The authors are supporters of the actively-temporizing tactics. They claim that the period of the delay in the decision should not exceed 24 hours and depends on the intensity of the bleeding, the degree of blood loss, the location and nature of the ulcer, the patient's age, and the concomitant diseases.

Among 322 patients with postvagotomy syndromes who were examined by the authors, 133 were subjected to reconstructive and corrective operations. Most of the patients (73.7%) who underwent surgery for the second time suffered from peptic, recurrent, and persisting ulcers due to vagotomy.
