Non-White Race and Concomitant Orthopedic Conditions Are Risk Factors for Failure to Achieve Clinically Relevant Improvement After Total Knee Arthroplasty.

Background: Data from the American Joint Replacement Registry demonstrate that 1-year minimal clinically important difference (MCID) achievement rates after total knee arthroplasty (TKA) are substantially lower when using general patient reported outcome measures, such as Patient-Reported Outcomes Measurement Information System Physical Function (PROMIS-PF), than joint specific measures. The purpose of this study was to evaluate patient characteristics and outcomes associated with MCID achievement after TKA using the PROMIS-PF measure.

Methods: A retrospective review of 263 patients undergoing TKA with preoperative and 1-year postoperative PROMIS-PF scores from March 12, 2020 to February 8, 2022 was performed.

Carotid endarterectomy without temporary intraluminal shunt. Study of 309 consecutive operations.

A prospective study was undertaken to determine the efficacy of performing carotid endarterectomy without an intraluminal shunt. During a two-year period, 240 patients, ranging in age from 36 to 89 years, underwent 309 consecutive carotid endarterectomies. The indication for operation was transient ischemic attacks in 151 (63%) patients, asymptomatic carotid bruit in 67 (28%), and previous stroke in 22 (9%).

Reoperation for recurrent coronary artery disease: causes, indications, and results in 168 patients.

Of the 13,049 patients who underwent coronary artery bypass surgery, 168 underwent a second or third operation because of the failure of the first. The early mortality was eight of 168. Good to excellent results were obtained in 141 patients.

A ten year study of heparin therapy for thrombophlebitis in ambulatory patients.

During the past ten years, we have observed 407 patients with thrombophlebitis using a standardized outpatient regimen including subcutaneously self-administered heparin therapy. A definite protocol for tapering and discontinuing anticoagulants was applied which allows a correlation between duration of heparin administration, decreasing heparin resistance and symptomatic improvement. In acute and subacute thrombophlebitis, this method induced symptomatic resolution within less than two months in half of the patients and within less than six months in 78%.

The haematological hazards of autotransfusion.

Fourteen heparinized dogs were autotransfused from 1-5 to 6 times calculated blood volume. Five animals retransfused 12 litres from an intraperitoneal bleed had marked decreases in all cellular elements, haematuria and a large (82 percent) drop in fibrinogen. All these animals died within 12 hours and post-mortem examination revealed evidence of disseminated intravascular coagulopathy.
