A 56-year-old woman initially noticed dizziness in October, 1988, and later dementia and gait disturbance developed, associated with myoclonus and periodic synchronous discharge in the electroencephalogram. On the basis of these clinical findings we made a diagnosis of Creutzfeldt-Jakob disease (CJD). Using RIA for ubiquitin (signal peptide of the ATP dependent proteolytic system), we measured the cerebrospinal fluid (CSF) ubiquitin levels.

The spinal cords from seven autopsy cases of sporadic motor neuron disease (MND) and two controls were immunohistochemically examined using antibodies directed to calcitonin gene-related peptide (CGRP) and to neurofilament proteins (Nf). CGRP immunoreactivity was observed in the posterior horns, especially in the laminae I and II, of all the spinal cords examined. In MND cases, in addition, a considerable number of spheroids in the anterior horns were labelled with the antibody.

A horizontal contrast medium margin crossing over the condyle (edge sign) and the relative size of the anterior recess of the lower joint compartment in the lateral and medial parts of the joints were used as indicators of the mediolateral position of the disk. Arthrographic findings were compared with coronal cryosections in a blinded fashion.

Nine adult rabbits were operated on bilaterally with the use of fibrin sealant on one joint and sutures on the other joint. The rabbits were killed after 3 months.

The arthrograms of 34 individuals with asymptomatic, clinically and arthrographically normal temporomandibular joints were analyzed. The results showed a substantial variation of the dimensions and configuration of the joint compartments. Thus the anterior recess of the lower joint compartment was small in 15 joints (44%), medium-size in 16 joints (47%), and large in three joints (9%).

The high condylar shave has the disadvantage of removing the articular soft tissue cover together with the underlying condylar bone. In an attempt to overcome this disadvantage, we elevated the articular soft tissue cover, shaved the condylar head, and replaced the articular soft tissue cover in its original position in nine adult rabbit temporomandibular joints. On the contralateral joints, the same amount of bone was shaved together with the articular soft tissue cover.

Unilateral temporomandibular joint arthrography was performed in 40 healthy volunteers with asymptomatic and clinically normal temporomandibular joints. Thirty-four joints (85%) showed superior disk position, and six joints (15%) were radiographically abnormal with displacement of the disk. Thus, two showed anterior displacement, one showed rotational anterolateral displacement, one showed rotational anteromedial displacement, one showed sideways lateral and one showed sideways medial displacement.

The anatomy and function of the temporomandibular joint are complex, and our understanding about how the retrocondylar tissues change when the condyle translates anteriorly has been incomplete. We therefore examined the sectional anatomy of 24 fresh human temporomandibular joints at different gradations of jaw opening, protrusion, and laterotrusion. We found that the tissues of the posterior disk attachment expanded in volume and occupied the space behind the condyle when the mouth was opened, protruded, or laterotruded to the contralateral side.

Arthoscopy was performed in the upper joint compartment of 28 temporomandibular joints in 16 fresh cadavers to investigate the accuracy of arthroscopic diagnoses of perforation and adhesion. Subsequent dissections revealed that only three of nine perforations were seen arthroscopically and that three of four joints with adhesions were arthroscopically diagnosed. There was no overdiagnosis of perforation or adhesions.

The surgical patients had internal derangement and severe long-standing temporomandibular joint pain and dysfunction. The normal disks were biconcave, whereas the surgically removed disks were deformed and thicker than the normal disks.

Rotational disk displacement implies a combination of anterior and medial or lateral displacements, whereas sideways displacement implies pure medial or lateral displacement without an anterior component. Multiple 3-mm-thick coronal and sagittal MR images were obtained of 18 fresh TMJ autopsy specimens and compared with the observations in corresponding coronal cryosections.

As a late complication after radical neck dissection, this paper reports a bony mass at the sternal end of the clavicle associated with hypertrophy of the sternohyoid muscle. The bony mass and hypertrophy of the muscle were considered due to continuous mechanical strain by the drooping of the shoulder caused by injury of the accessory and cervical nerves, left-handed physical labour, and the lack of the sternocleidomastoid muscle in rotating the head.
