Background: Left bundle branch area pacing (LBBAP) has the advantages of narrow QRS duration, rapid peak left ventricular (LV) activation, and LV dyssynchrony correction with a low, stable pacing output. Here we report our experience with patients undergoing LBBAP with a left bundle branch block (LBBB) for clinically indicated pacemaker or cardiac resynchronization therapy implantation. We compared the initial follow-up data of these patients and patients undergoing conventional right ventricular pacing (RVP).

Article Synopsis
  • The Improve SCA trial highlights the underutilization of implantable cardioverter-defibrillators (ICDs) in regions like Asia, Latin America, and Africa, showing their potential benefits for patients with ischemic (ICM) and non-ischemic cardiomyopathy (NICM) who are at risk of sudden cardiac arrest (SCA).
  • Out of 1848 non-ischemic and 581 ischemic cardiomyopathy patients, those who received ICDs had significantly lower all-cause mortality rates at 3 years compared to those without the devices, indicating a strong mortality benefit.
  • The study concludes that ICD implantation is crucial for improving survival in primary prevention patients and that the rate of appropriate device therapy is

Impaired left atrial appendage ejection fraction (LAA-EF) and peak LAA flow velocity (LAA-FV) are associated with high thromboembolic risks in patients with atrial fibrillation (AF). Herein, we examined LAA function among patients with atrial flutter (AFL) stratified by the CHADS-VASc score using transesophageal echocardiography (TEE). Of 231 consecutive patients with typical AFL, 84 who fulfilled the inclusion criteria were enrolled.

Aims And Background: Idiopathic ventricular fibrillation (IVF) is a cause of sudden cardiac death (SCD). The frequency of mutations in disease-causing genes ranges, on average, between 16 and 48% in SCD cases. This study aimed to identify novel mutations in IVF patients without , and mutations using whole-exome sequencing (WES).

QRS duration has been associated with the response to cardiac resynchronization therapy (CRT). However, the methods for defining QRS duration to predict the outcome of CRT have discrepancies in previous reports. The aim of this study was to determine an optimal measurement of QRS duration to predict the response to CRT.

Background: Familial sick sinus syndrome is associated with gene mutations and dysfunction of ion channels. In contrast, degenerative fibrosis of the sinus node tissue plays an important role in the pathogenesis of acquired sick sinus syndrome. There is a close relationship between transforming growth factor-β1 mediated cardiac fibrosis and acquired arrhythmia.

Article Synopsis
  • * Researchers screened 113 SSS patients and 125 controls for specific genetic polymorphisms related to angiotensinogen (AGT) and connexin 40 (Cx40), finding a significant link between certain AGT promoter polymorphisms and the disease.
  • * The findings suggest that the G-6A polymorphism in the AGT promoter impacts gene expression and may increase susceptibility to non-familial SSS, highlighting the importance of genetic factors in the condition's development.

Single-beat determination of left ventricular systolic function at a beat with equal subsequent cardiac cycles has been proposed as an accurate method in atrial fibrillation. However, there has still been substantial variability between the values calculated from beats with equal subsequent cycles. Therefore, some refinement on the single-beat method is needed.

Idiopathic left ventricular tachycardia (ILVT) is a distinct entity that arises in the left ventricle, may have reentrant mechanism and is verapamil-sensitive. Pleomorphism as defined by multiple ventricular tachycardia morphologies is usually associated with either coronary artery disease or cardiomyopathy but very rare in cases of ILVT. In this case report, we describe an unusual case of ILVT with two ECG morphologies of the opposite axis that were successfully eliminated with radiofrequency ablation.
