Based on his experience of treatment of more than 5000 diabetic patients with surgical diseases, the author describes basic principles of management of these patients before, during and after operation. Combined treatment is based on compensation of diabetes, fluid-and-electrolyte balance and acid-base equilibrium. Blood glucose before operation must be limited to 8.

The authors examined 34 patients with diabetes mellitus and purulent diseases of the soft tissues and 40 persons who formed the control group. The study of local microcirculation in the purulent focus, in a purulent and closed wound by determining the oxygen tension in capillary blood (pO2) established by using an noninvasive method that a real possibility for determining the exact boundaries of tissue necrosis has appeared. This makes it possible to excise optimally the necrotic tissues and choose the techniques of applying primary, primary-delayed or early secondary sutures.

The authors suggest using the regional blood flow in control over the course of the wound process after various types of laparotomy so as to prognosticate the possibility of suppuration of the postoperative wounds. Examination of 20 patients showed that the regional blood flow values depended on the operative trauma and the character of the inflammatory process in the abdominal cavity. Decrease of the regional blood flow in the wound to 24 ml/min/100 g and lower indicates that purulent inflammation will most probably develop in the postoperative wound.

The work presents data on treatment of 28 patients with neclostridial anaerobic infection developed against the background of diabetes mellitus. It was shown that characteristics clinical symptoms of the ailment and express-microscopic examination of the native material from the wound facilitated making the correct diagnosis in most cases without using complicated microbiological and chromatographic investigations.
