Paragangliomas are uncommon neck swellings, usually associated with hypertension and flushing. They occur in the neck and other amine precursor uptake and decarboxylation cell rests, and are a challenge to the anesthetist at the time of excision.

Chronic nonhealing ulcer of the lower limb with mixed arterio-venous pathology.

Chronic wounds in which arterial and venous pathology coexist are relatively uncommon. The authors report a case of a diabetic lady who presented with claudication pain and a chronic nonhealing lower extremity wound of more than 6 months. In these patients, it is mandatory to treat the arterial insufficiency first, and it is also important to avoid compression bandaging, which is advised for venous ulcers.

Efficacy and safety of cilostazol, a novel phosphodiesterase inhibitor in patients with intermittent claudication.

Pharmacotherapy is limited for the relief of intermittent claudication (IC), a common manifestation of peripheral arterial disease (PAD). Pentoxyfylline, the only current pharmacological therapy for IC, has been shown to have similar efficacy as placebo. Cilostazol, a new phosphodiesterase III (PDE III) inhibitor, is a potent inhibitor of platelet aggregation with vasodilatory, antithrombotic, antiproliferative and positive lipid-altering effects.
