Objectives: To provide one of the first prospective reports examining neuropsychological outcomes for children treated with 1800 cGy whole brain radiotherapy (WBRT) and prophylactic chemotherapy versus prophylactic chemotherapy alone for acute lymphoblastic leukemia (ALL). The purpose of this study was to identify characteristics associated with health-related quality of life (HRQOL) among long-term survivors of adolescent cancer enrolled in the Childhood Cancer Survivor Study. Thirty percent of survivors reported poor physical and/or mental HRQOL. Race/ethnicity, education, and head/neck disfigurement were significantly associated with poor mental HRQOL, while sex, age, household income, obesity, alkylating agents, pelvic radiation, head/neck or limb disfigurement, and walking with a limp were associated with poor physical HRQOL.

Purpose: Methotrexate (MTX) can cause significant clinical neurotoxicity and asymptomatic leukoencephalopathy. We sought to identify clinical, pharmacokinetic, and genetic risk factors for these MTX-related toxicities during childhood acute lymphoblastic leukemia (ALL) therapy and provide data on safety of intrathecal and high-dose MTX rechallenge in patients with neurotoxicity.

Objectives: To summarize the current knowledge about 1) cognitive changes associated with central nervous system-directed chemotherapy and cranial radiation among children with acute lymphoblastic leukemia and brain tumors and adult survivors; and 2) interventions designed to prevent or remediate the cognitive and academic problems associated with central nervous system-directed cancer treatment.

Data Sources: Classic and current databased publications.

Conclusion: Future directions for research include 1) identification of sources of variability in long-term outcomes; 2) greater understanding of the developmental evolution of deficits across the survivor lifespan; and 3) interventions to treat and prevent negative outcomes following cancer therapy.

The survival rate for childhood acute lymphoblastic leukemia (ALL) is greater than 80%. However, many of these survivors develop long-term chronic health conditions, with a relatively common late effect being neurocognitive dysfunction.
