Introduction: Arterial cannulation is frequently performed on intensive care unit (ICU) and operating room patients; a 1% complication rate has been reported. Investigators applied simulation to study clinical providers' arterial catheter (AC) insertion performance and to assess for interdisciplinary and intradisciplinary variation that may contribute to complications.

Methods: Anesthesia, medical critical care, and surgical critical care providers with AC insertion experience were enrolled at 2 academic hospitals.

Objective: Protective lung ventilation is reported to benefit patients with acute respiratory distress syndrome. It is not known whether protective lung ventilation is also beneficial to patients undergoing single-lung ventilation for elective pulmonary resection.

Methods: In an institutional review board-approved prospective randomized trial, 34 patients undergoing elective pulmonary resection requiring single-lung ventilation were enrolled.

Acute lung injury/acute respiratory distress syndrome is a syndrome of low respiratory compliance. However, longstanding knowledge of applied respiratory mechanics and refined imaging techniques have shown that this is clearly an oversimplified view. Though the average compliance of the respiratory system is reproducibly low, regional mechanics may vastly differ; lung, airway, and chest wall mechanics may be variably affected; finally, these abnormalities may be very dynamic in nature, being influenced by time, posture, and the way positive-pressure ventilation is applied.
