Introduction: Synchronous liver metastases occur in approximately 15-20% of patients with colorectal cancer. Optimal oncological treatment of oligometastatic disease combines surgical resection and systemic therapy. Open simultaneous resection of the primary and liver metastases is well described, but there is not much evidence for the increasing use of the minimally invasive approach.

Article Synopsis
  • Multimodal two-stage hepatectomy (mTSH) is a surgical approach for patients with bilobar colorectal liver metastases (CRLM) who can't be treated with a single operation due to limited liver function.
  • A narrative review of clinical studies shows that interval chemotherapy is used in mTSH and might improve disease control, though only a few studies have specifically reported its effects.
  • Current data suggest that interval chemotherapy does not negatively impact liver growth during mTSH or lead to serious complications, but more randomized clinical trials are needed for definitive conclusions.

Article Synopsis
  • Intrahepatic cholangiocarcinoma (iCC) is an aggressive tumor often diagnosed late, prompting this study to evaluate body composition analysis (BCA) from CT scans for predicting surgical complications.
  • The research involved 86 patients and found that sarcopenia (muscle loss) and higher levels of subcutaneous and visceral fat increased the risk of complications, leading to longer hospital and ICU stays.
  • The study concluded that BCA is more effective than BMI for assessing risks before surgery in iCC patients, suggesting its incorporation into routine preoperative evaluations.

Risk prediction prior to oncologic esophagectomy is crucial for assisting surgeons and patients in their joint informed decision making. Recently, a new risk prediction model for 90-day mortality after esophagectomy using the International Esodata Study Group (IESG) database was proposed, allowing for the preoperative assignment of patients into different risk categories. However, given the non-linear dependencies between patient- and tumor-related risk factors contributing to cumulative surgical risk, machine learning (ML) may evolve as a novel and more integrated approach for mortality prediction.

Background: Colorectal cancer is the third most common tumour entity in the world and up to 50% of the patients develop liver metastases (CRLM) within five years. To improve and personalize therapeutic strategies, new diagnostic tools are urgently needed. For instance, biomechanical tumour properties measured by magnetic resonance elastography (MRE) could be implemented as such a diagnostic tool.

Purpose: Minimal-invasive liver surgery (MILS) reduces surgical trauma and is associated with fewer postoperative complications. To amplify these benefits, perioperative multimodal concepts like Enhanced Recovery after Surgery (ERAS), can play a crucial role. We aimed to evaluate the cost-effectiveness for MILS in an ERAS program, considering the necessary additional workforce and associated expenses.

Nicotinamide adenine dinucleotide (NAD), a coenzyme for more than 500 enzymes, plays a central role in energy production, metabolism, cellular signaling, and DNA repair. Until recently, NAD was primarily considered to be an intracellular molecule (iNAD), however, its extracellular species (eNAD) has recently been discovered and has since been associated with a multitude of pathological conditions. Therefore, accurate quantification of eNAD in bodily fluids such as plasma is paramount to answer important research questions.
