We studied a total of 177 patients with cervical intraepithelial neoplasia or invasive carcinoma to evaluate the clinical implications of a positive endocervical curettage. All patients underwent cervical conization. One hundred and ten women had unsatisfactory colposcopy; 67 had satisfactory colposcopy.

Following complete ablation of the lesions by one or more treatment modalities, 45 patients applied a single dose of 5% 5-fluorouracil cream biweekly to the vagina and/or vulva for at least six months. Forty-five patients used no prophylactic 5-fluorouracil cream.

Of all lacerations, 37% occurred during entrance into the peritoneal cavity, 35% during lysis of adhesions or performance of pelvic or abdominal dissections, 10% during laparoscopy, 9% during vaginal operations, and 9% during dilatation and curettage and dilatation and evacuation. Injury involved the small intestines in 75% and the large intestines in 25% of the cases.

View Article and Find Full Text PDFThirty-three healthy women (group 1), 20 patients with a history of ovarian carcinoma but no manifest disease at the time of the study (group 2), and 45 patients with surgically demonstrable ovarian cancer (group 3) were studied to establish guidelines for the use of the ovarian cancer antigen Ca 125 in monitoring the course of ovarian carcinoma. Ninety-nine percent of all Ca 125 titers of patients in groups 1 and 2 were less than or equal to 25 U/mL. By contrast, 96% of patients with manifest ovarian cancer had Ca 125 levels greater than 25 U/mL.

We retrospectively studied 11 patients with flat condylomas confined to the cervix; all lesions were eradicated by conventional therapy. Three patients studied retrospectively and 16 studied prospectively had condylomatous lesions involving several sites within the lower genital tract.

The technique described herein is simple, fast and provides a secure anastomosis. Mobilization and extensive dissection of either the Hartmann pouch or the stump of the sigmoid colon is avoided. In addition, there is no need for pursestring sutures, which may be difficult to place in patients with short rectal stumps.

A combination of cis-platinum, doxorubicin (Adriamycin), and methotrexate was used to treat 28 patients with recurrent cervical cancer. Seven patients achieved a partial response with a median duration of ten months. Two patients achieved a complete response with a median duration of 14 months.

View Article and Find Full Text PDFThe medical records of 48 patients who underwent laser vaporization of therapy-resistant or recurrent condylomata acuminata were studied with regard to history of immunosuppression, location of original lesion, type and duration of therapy, and whether magnification devices were used prior to or during therapy. The genitalia of the patients' sexual partners were examined in all cases in which the condylomata recurred after initial control of the lesions with conventional methods. In 46 of the 48 cases, possible reasons for the failure of conventional therapy could be found: unresponsive to chemical therapy (11 patients); reinfection by the sexual partner (4); immunosuppression (8); several factors (6); and incomplete eradication of the lesion (20).

Patients at high risk of treatment failure were divided into three groups: (I) CIN grade 3 lesions confined strictly to the ectocervix (86 patients), (II) CIN grade 1-3 lesions involving the endocervix to a depth of up to 5 mm and a negative ECC prior to cryotherapy (64 patients), and (III) CIN grade 1-3 lesions confined to the ectocervix or extending into the endocervix to a depth of up to 5 mm and a positive ECC (27 patients). A fourth group, 68 patients with CIN grade 1 and 2 lesions confined to the ectocervix, was considered to be at low risk of treatment failure and served as controls.

View Article and Find Full Text PDFBecause of an unsatisfactory colposcopic examination, cold knife conization of the cervix is frequently required for the evaluation of postmenopausal patients with abnormal cervical cytology. With advancing age, the cervix atrophies and retracts so that conization becomes increasingly difficult and results in a tissue specimen that is frequently inadequate. Dissatisfied with the results of the standard conization procedure, the authors performed partial trachelectomy in 26 elderly patients with atrophic, retracted cervixes.

Herniation of the bladder has been primarily described in association with inguinal and femoral hernias. Review of the literature yielded only one case report of incisional bladder hernia. The authors report three cases of herniation of the bladder through a postoperative fascial defect.
