The process of stone formation in the human body remains incompletely understood, which requires clinical and laboratory studies and the formulation of a new endogenous, nanotechnological concept of the mechanism of origin and formation of crystallization centers. Previously, the mechanism of sialolithiasis was considered a congenital disease associated with the pathology of the ducts in the structure of the glands themselves. To date, such morphological changes of congenital nature can be considered from the position of the intrauterine formation of endogenous bacterial infections complicated by the migration of antigenic structures initiating the formation of crystallization centers.

[Complications and effectiveness of treatment of patients with locally advanced prostate cancer after combined radiotherapy and radical prostatectomy with postoperative radiotherapy].

Treatment for prostate cancer remains a significant social problem due to the continuing trend of growth of morbidity and mortality in Russia from this disease. In recent years a real alternative to surgical treatment is radiotherapy. In treatment of locally advanced stages of prostate cancer radiotherapy plays a dominant role.

[The role of magnetolaserotherapy in the correction of the adaptive potential of the brain in the children suffering absence seizures].

We have conducted a course of magnetic laser therapy targeted on the vegetative structures at the neck of the children suffering absence seizures in an attempt to optimize the functioning of the non-specific brain structures. The study has demonstrated that such treatment promotes normalization of the components of the orientation response to sound almost to the level observed in the healthy children. The alpha-index returned to the normal value as well.

[Results of the spectral analysis of electroencephalograms of patients with focal epileptic seizures in the interictal period].

A comparative spectral analysis of electroencephalograms in patients with focal and generalized epileptic seizures in the interictal period revealed some specific characteristics of these paroxysms. Slowing of dominating alpha-rhythm was seen in case of the localization of epileptiform activity in frontal-central-temporal and occipital-parietal areas with the further spreading of epileptiform activity to the temporal area. Asymmetry of dominating alpha-rhythm in the right hemisphere was found in patients with focal paroxysms, asymmetry of theta-rhythm in all patients, of beta-2 rhythm in patients over 45 years old and of delta-2 rhythm in female patients.

Overlapping and distinct roles of Aspergillus fumigatus UDP-glucose 4-epimerases in galactose metabolism and the synthesis of galactose-containing cell wall polysaccharides.

The cell wall of Aspergillus fumigatus contains two galactose-containing polysaccharides, galactomannan and galactosaminogalactan, whose biosynthetic pathways are not well understood. The A. fumigatus genome contains three genes encoding putative UDP-glucose 4-epimerases, uge3, uge4, and uge5.

Aspergillus galactosaminogalactan mediates adherence to host constituents and conceals hyphal β-glucan from the immune system.

Aspergillus fumigatus is the most common cause of invasive mold disease in humans. The mechanisms underlying the adherence of this mold to host cells and macromolecules have remained elusive. Using mutants with different adhesive properties and comparative transcriptomics, we discovered that the gene uge3, encoding a fungal epimerase, is required for adherence through mediating the synthesis of galactosaminogalactan.

[The membrane stabilizing effect of glatiramer acetate in multiple sclerosis].

The effect of glatiramer acetate on the structure of the lipid phase of cell membranes of red blood cells, the status of lipid peroxidation and antioxidant defense system in patients with relapsing forms of multiple sclerosis. It is shown that during treatment with glatiramer acetate reduces the initially high, level of phospholipase A2, and the concentration of lipid peroxidation products - diene conjugates and Schiff bases. At the same time against the increase initially reduced activity of the antioxidant defense normal structure of erythrocyte membranes in patients with relapsing-remitting multiple sclerosis.

[Dysregulation of autonomic, hypophysis-adrenal and immune systems in patients with severe brain injury].

Thirty-three patients were studied in the acute stage of severe brain injury, 18 patients died during 24 days in a hospital, 15 patients had survived. The functional status of autonomic, hypophysis-adrenal and immune systems was followed up using variation cardiointervalography, Adrenocorticotropic hormone (ACTH) and cortisol as well as cell immunity parameters were measured. In patients with a fatal outcome, the overstrain of compensatory mechanisms of autonomic supply (mode amplitude, AMo and index of strain, IS) noted in the first day was exhausted to 19-22 days.

Urologists' attitudes regarding information sharing with prostate cancer patients--is there a common ground for collaboration with family physicians?

Clinicians often fail to adequately meet prostate cancer patients' information needs, and patients may receive different kinds of information from their doctors. This study aims to describe urologists' attitudes regarding information sharing with prostate cancer patients and to compare these findings with the previously published attitudes of Israeli family physicians. A questionnaire (11 items) was mailed to 87 board-certified practicing urologists.

[Somatosensory evoked potentials in diagnosis of chronic cerebral ischaemia in patients with pathology of brachiocephalic arteries].

The present study was aimed at substantiating the practical significance of using the method of the somatosensory evoked potentials (SSEPs) in order to evaluate the functional state of the brain in patients suffering from chronic cerebral ischaemia (CCI) on the background of lesions of the brachiocephalic arteries (BCAs). With the method of the SSEPs, prior to reconstructive operation on the BCAs we examined a total of three hundred and seventeen 31-to-82-year-old patients. Of these, 156 patients were diagnosed as having a clinically symptom-free course of CCI, and the remaining 161patients appeared to have clinically symptomatic CCI.

[Dynamics of parameters of autonomous regulation, humoral immunity and neuroendocrine system in acute brain injury with fatal outcome].

Twenty-two patients with brain injury who died within 24 days after admission to the hospital were studied. Autonomous regulation was studied using a method of variation cardiointervalography (VCIG), humoral immunity - by a method of gel radial immunodiffusion, contents of adrenocorticotropic hormone (ACTH) and hydrocortisone - by a radioimmune method. VCIG registration and blood sampling were carried out in the morning in 1-3, 4-6, 7-10, 11-14, 15-18 and 19-22 days after admission to the hospital.

[Nonspecific brain systems and rehabilitation of children with cerebral palsy].

Basing on a long-term investigation of 157 children with spastic forms of cerebral palsy, regressive mathematical models of motor development have been worked out, taking into account a degree of severity and outcome of the disease. Mathematical patterns, reflecting dysneuroontogenesis of suprasegmental regulation system, are obtained. Complex electroencephalographic, electromyographic and vegetologic data demonstrated a role of limbic and reticular complex of intersystemic and intrasystemic disintegration in the reduction of adaptability and rehabilitation potential in patients with cerebral palsy.

[Results of day polygraph study in children with spastic forms of cerebral palsy].

Integrative possibilities of non-specific brain systems of 30 patients with cerebral palsy (CP) aged 6-10 years were studied in the process of day polygraph examination including the record of EEG, skin-galvanic reflex and ECG both at physiological rest and at functional trials. 14 children had spastic diplegia, 8 children--hemiparetic form on the right side and 8 patients--on the left. The results obtained have indicated a systemic disneuroontogenesis of the middle structures mainly with the intrasystemic, intersystemic and interhemispheric manifestations of disintegration.

[Use of absorbable polymers for plastic repair of postoperative ventral hernias].

In surgical clinic of Vladivostok State Medical University 48 patients were operated for postoperative ventral hernias with use of absorbable polymers. In group 1 (42 patients) the explant "Carbonicus-I" was used (period of absorption-2 years). 6 patients of group 2 underwent plastic repair with suture-glue duplication of aponeurosis of abdominal white line.

[The status of adaptive-compensatory mechanisms after the brain concussion in children].

80 children of preschool age were observed after concussion of the brain. In follow-up from 6 months 3 years clinical-anamnestic and neuropsychologic methods were used. The functions of autonomic nervous system were evaluated according to the results of estimation of initial autonomic tonus, autonomic reactivity, autonomic maintenance of the activity and the data of cardiointervalography.

[Plastic surgery of the anterior abdominal wall in children using synthetic resolving materials].

The data on the first experience in the use of synthetic resolvable materials in plasty of the anterior abdominal wall in children are reported. Tissue glue compositions based on alpha-cyanoacrylates as well as connective biocompatible elements (CBE) developed at the SRIIMT were used. An aponeurotic suture-glue duplication was utilized for plastic enforcement of the anterior abdominal wall; individual methods were developed for each concrete localization of the defects.

[Night sleep in temporal-lobe epilepsy in children].

Polygraphy of night sleep performed in 31 children with temporal epilepsy showed enhanced nonspecific mesolimbic activation, probably of compensatory nature, in patients with common partial fits or complex partial fits. In wakeful patients with complex partial fits there were signs of growing influence of synchronizing inactivating non-specific structures.

[The adaptive systems of the brain in typical absence seizures in children during treatment].

Combined investigation of vegetative regulation in children with absences revealed overstrain of the sympathetic vegetative nervous system. This underlies the use of sinusoidal modulated currents (SMC) in multimodality treatment of the disease. SMC are applied on the cervical vegetative formations.

[Esthetic effects of surgery in hernia of the anterior abdominal wall in children].

The cosmetic effect of operative interventions for the anterior abdominal wall hernia, vaginal process of peritoneum anomalies in 558 patients aged from 7 to 27 years was studied. The 15-year follow-up was available. Poor cosmetic results appropriateness were determined, the advantages of the sutureless abdominal wound closure were examined, an operative accesses for the umbilical hernia and the hernia of linea alba treatment were proposed.

[The use of a tissue adhesive in the surgical treatment of edema of the testicular membranes and the spermatic cord].

A method of operative treatment of scrotal and funicular hydrocele adhesion is described which provides the adhesion of the patent vaginal process of the peritoneum with the tissue cyanacrylate glue. Operations by the method of funiculocele adhesion were performed on 41 children of 1-13 years of age followed by good immediate and long-term results. No recurrences were noted.
