Background: Next-generation sequencing (NGS) is essential in treating advanced lung cancer. However, the effectiveness of endoscopic ultrasound with bronchoscope-guided fine-needle aspiration (EUS-B-FNA) in NGS remains unclear. This study examined the usefulness of EUS-B-FNA in lung cancer NGS cases where EUS-B-FNA was performed for specimen submission in a nationwide genomic screening platform (LC-SCRUM-Asia) and compared specimens collected using other bronchoscopy methods (endobronchial ultrasound-guided transbronchial needle aspiration [EBUS-TBNA] and EBUS-guided transbronchial biopsy with a guide sheath [EBUS-GS-TBB]) during the same period.

Article Synopsis
  • The study examined the use of C-reactive protein to albumin ratio (CAR) as a prognostic marker in adult T-cell leukemia-lymphoma (ATL), analyzing data from 68 patients diagnosed between 2013 and 2017.
  • Results showed that higher CAR levels were linked to worse overall survival, with patients having a CAR above 0.553 surviving a median of 3.94 months compared to 8.37 months for those below this threshold.
  • The findings suggest that CAR could serve as a simple and valuable independent prognostic marker, particularly in patients undergoing chemotherapy.

Article Synopsis
  • The case involves a 67-year-old male with anemia and significant splenomegaly, where splenomegaly is a key sign of primary myelofibrosis but can also be confused with other conditions like malignant lymphoma.
  • Laboratory tests showed HTLV-1 antibodies, elevated interleukin levels, and abnormalities in bone marrow and spleen imaging, prompting a laparoscopic splenectomy to relieve symptoms and rule out malignancy.
  • Histological analysis revealed extramedullary hematopoiesis, inflammation, and specific mutations, leading to a diagnosis of primary myelofibrosis, which subsequently reduced the patient's need for blood transfusions post-surgery.

Primary bronchial tumors are extremely rare. However, symptoms, such as coughing and wheezing, are not specific to this disease, and primary bronchial tumors are often misdiagnosed as bronchial asthma. This report describes the case of a pregnant patient with a bronchial tumor that mimicked deteriorating bronchial asthma.

Invasive mucinous adenocarcinoma (IMA), which is a relatively rare lung adenocarcinoma, is considered a high-grade subtype and is associated with a poor prognosis. IMA is difficult to diagnose by computed tomography because it requires differentiation from inflammatory diseases, such as atelectasis, infectious pneumonia, and organizing pneumonia. Thus far, no reports of radial endobronchial ultrasonography (EBUS) findings in IMA have been published.

Clozapine is the most effective antipsychotic medication for refractory schizophrenia, but it has many possible serious side effects, including antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV). However, the rare case reports available have not presented sufficient characteristic features of drug-induced AAV. Herein, we report a case of a 48-year-old Japanese woman with schizophrenia who presented with fever, arthralgia, myalgia and skin rash after 2 years of clozapine treatment.

Article Synopsis
  • The study investigates the effects of a humanized CCR4 antibody called mogamulizumab on patients with aggressive adult T-cell leukemia-lymphoma (ATL), analyzing data from 22 patients treated from 2012 to 2018.
  • Results showed an overall response rate of 68.1% after 4 treatment courses, but the median overall survival was only 95.5 days, with 6 and 12-month survival rates at 31.5% and 21.1%, respectively.
  • Patients with CCR4 mutations exhibited a poorer response to treatment compared to those with wild-type CCR4, suggesting a potential link between CCR4 expression and treatment efficacy, warranting further research to identify more predictive markers.

A new entity, namely early/prefibrotic primary myelofibrosis (PMF), was introduced as a subtype of PMF in the 2016 revised World Health Organization (WHO) criteria for myeloproliferative neoplasms (MPN). It was diagnosed based on histopathological features of bone marrow (BM) biopsy specimens together with clinical parameters [leukocytosis, anemia, elevated lactate dehydrogenase (LDH) values, and splenomegaly]. The aim of this study was to evaluate the prevalence of early/prefibrotic PMF in patients who were previously diagnosed with ET, and to compare clinical features at diagnosis and outcomes between early/prefibrotic PMF and essential thrombocythemia (ET) patients.

Objectives: It can be difficult to differentiate diffuse malignant peritoneal mesothelioma (DMPM) from reactive mesothelial hyperplasia (RMH) or peritoneal dissemination of gynecologic malignancies, such as epithelial ovarian cancer (EOC), which cause a large amount of ascites. Detection of the homozygous deletion of p16/CDKN2A (p16) by fluorescence in situ hybridization (FISH) is an effective adjunct in the diagnosis of malignant pleural mesothelioma. The aim of this study was to investigate the ability of the p16 FISH assay to differentiate DMPM from RMH and EOC.
