Objective: Knowledge of hostile factors and their influence on long-term seal in the iliac landing zone is limited. Currently endorsed clinical practice guidelines lack structural evidence on how the iliac landing zone should be assessed in the pre-, intra-, and postoperative phases. The goal of this study was to obtain an international, expert-based consensus on the definition of a hostile iliac landing zone, on how to size and plan stent-grafts to optimize sustainable distal seal, and on the postprocedural follow-up protocol.

Article Synopsis
  • Nutritional care is seen as essential for improving outcomes in patients with chronic limb threatening ischemia (CLTI) due to their high risk of complications.
  • A qualitative study involving focus groups revealed that healthcare professionals acknowledge the importance of nutritional care but often give it insufficient attention due to various barriers.
  • Key challenges identified include knowledge gaps, a lack of routine incorporation of nutritional care, patient-related difficulties, and time constraints; however, enhancing scientific knowledge and organizational processes can help facilitate better nutritional care.

Background: Although breast reconstruction in the setting of post-mastectomy radiotherapy (PMRT) is controversial, we offer nipple-sparing mastectomy and immediate implant-based breast reconstruction ([N]SSM/IIBR) to women needing primary mastectomy regardless of PMRT. Nevertheless, some of these women have no reconstruction.

Purpose: To assess the uptake of breast reconstruction in women who undergo PMRT and the patient characteristics associated with such uptake.

The Dutch national open database on COVID-19 has been incrementally expanded since its start on 30 April 2020 and now includes datasets on symptoms, tests performed, individual-level positive cases and deaths, cases and deaths among vulnerable populations, settings of transmission, hospital and ICU admissions, SARS-CoV-2 variants, viral loads in sewage, vaccinations and the effective reproduction number. This data is collected by municipal health services, laboratories, hospitals, sewage treatment plants, vaccination providers and citizens and is cleaned, analysed and published, mostly daily, by the National Institute for Public Health and the Environment (RIVM) in the Netherlands, using automated scripts. Because these datasets cover the key aspects of the pandemic and are available at detailed geographical level, they are essential to gain a thorough understanding of the past and current COVID-19 epidemiology in the Netherlands.

Article Synopsis
  • The research investigates whether the 'immigrant health paradox,' where immigrants generally have better health outcomes, also applies to the birth weights of their children compared to native-born children in Western Europe and Australia.
  • Data from five major studies with thousands of participants was used to compare birth weights of infants born to immigrant parents versus native parents.
  • Findings revealed mixed results: children of immigrants in France and Australia had higher birth weights, while those in the UK and the Netherlands had lower birth weights, with smoking during pregnancy identified as a possible factor influencing these differences.

Background: Literature is scarce on the course of iliac endograft limb apposition after endovascular aortic aneurysm repair (EVAR), which is why this study was conducted.

Methods: A retrospective observational imaging study was performed to measure iliac apposition of endograft limbs on the first post-EVAR computed tomography angiography (CTA) scan and on the latest available follow-up CTA scan. With center lumen line reconstructions and CT-applied dedicated software, the shortest apposition length (SAL) of the endograft limbs was assessed as well as the distance between the end of the fabric and the proximal border of the internal iliac artery or the endograft-internal artery distance (EID).

Postmastectomy radiotherapy (PMRT) is allegedly associated with a higher risk of complications of combined nipple-sparing or skin-sparing mastectomy and subpectoral direct-to-implant immediate breast reconstruction ([N]SSM/SDTI-IBR). For this reason, this combination is usually advised against or, even, refused in women who need to undergo PMRT. Because this advice has never been justified, we assessed the short-term complications that may potentially be associated with PMRT after [N]SSM/SDTI-IBR.

Background And Aim: To date, studies on adjuvant chemotherapy as a risk factor for the surgical outcome of combined mastectomy and breast reconstruction were hampered by the inclusion of mixed reconstructive cohorts of both delayed and immediate timing and of both autologous and implant-based techniques. Consequently, there is a paucity of data on the impact of adjuvant chemotherapy on surgical complication rates after combined skin-sparing mastectomy and immediate implant-based breast reconstruction.

Methodology: We compared the postoperative complications that occurred within 16 weeks after this combined procedure in 131 women (139 breasts) treated with adjuvant chemotherapy with those in a control group of 491 women (517 breasts) not receiving any adjuvant therapy within 16 weeks.

Article Synopsis
  • Birth weight variation is affected by both genetic and non-genetic factors from the mother and fetus, influencing long-term health risks like cardio-metabolic issues.
  • A comprehensive analysis involving over half a million participants found 190 genetic signals related to birth weight, with many being newly identified.
  • The study suggests that while maternal genetics can lower a child's birth weight, this does not directly cause higher blood pressure later; instead, genetic factors play a key role in this relationship.
