Revascularizing osteotrepanation (ROT) was performed in 44 patients with chronic obliterating diseases of the lower extremity arteries. The modified technique consisted in putting opertures through and through both cortical plates but in the metaphyseal zones of the tibial bone, the muscles were not damaged, fascia was not sutured. A comparison was made of the data of local skin electrothermometry, index of regional systolic pressure (IRSP) on the femur, malleolus-humeral index (MHI) and the distance of painless walking before operation and on the 10th day.

The known methods of operative treatment of femur by means of intraosseous osteosynthesis, based on jamming of intraosseous structure in the bone, don't ensure sufficient level of the osteosynthesis stability that results in the secondary displacements. According to the method, proposed by the authors, a rod is introduced through the intercondylar fossa of the distal fractured fragment of femur (Imamaliyev, 1987) and is pierced through into the proximal fractured fragment of femur so that it perforates cortical layer of the bone in the region of greater trochanter with strong fixation in it. The structure is removed through the small incision in the area of the greater trochanter of femur.

The operation has anatomical and experimental grounds. Results of treatment of 6 patients have shown its high effectiveness in disturbed processes of consolidation of fractures of the upper and medial thirds of the femur diaphysis.

A clinical analysis of treatment of 328 diaphyseal fractures of long bones in 304 patients by functional intraosseous osteosynthesis which is not to be followed by plaster immobilization has shown that the function was reestablished long before the consolidation. The functional intraosseous synthesis was performed without drilling of the bone marrow cavity, without special appliances or devices. Terms of invalidism are shorter than traditional ones.
