[The issues of population reproductive health in Voronezhskaya oblast and means of its improvement].

The article deals with the results of analysis of demographic indicators of natural migration of population in Voronezhskaya oblast in 1999-2008 revealing particular characteristics. The state of reproductive health of population in oblast is characterized by stable preservation of lower level of birth rate, higher indicators of total mortality and negative natural growth of population. The allotment of "maternal capital" at the birth of second child from 2006 and increase of one-time dowry at child birth in the Voronezh region ceased the birth-rate falling and mapped out a growth trend.

[The medical social risk factors of developing the iron-deficiency anemia in pregnant women].

The results of study of medical social risk factors of developing the iron-deficiency anemia in pregnant women are presented. The medical case history data, the characteristics of pregnancy, delivery and puerperal period course, the indicators of laboratory and instrumental findings are discussed in their dynamics. The analysis of significance of risk factors is carried out, the results of comparing of parameters of basic and control groups according Student's test were used.

[Subtotal and total colectomy with formation of primary anastomosis for left colonic ileus of tumoral genesis].

The one-staged subtotal/total colectomy with formation of primary ileo-sigmoanastomosis or ileo-rectal anastomosis is applied in tumoral left colonic ileus. There were estimated immediate results and the role of the method in the treatment of 46 patients with left colonic obstructive tumor. Postoperative complications had occurred in 52% of the patients, postoperative mortality had constituted 10.

[An increase in the effectiveness of outpatient care of patients with kidney diseases].

Organization of medical care of patients with renal diseases in a municipal health center is described. The authors demonstrate the efficacy of specialized care when it was made available for population. The detection of renal diseases improved.

[International humanitarian aid in the control of tuberculosis in Voronezh Province].

Presents the experience gained in realization of a program of humanitarian care aimed at tuberculosis control in the Voronezh Province of Russia, which was organized by the European Community within the framework of UNO-EEC Echo-Rus/B7-515/93-1806B with participation of the Rudolf Virchow Free University of Berlin. Regular bilateral contacts, well-defined purpose of the program, permanent supervision from both sides over the fulfillment of plans and conditions of the agreement, direct supplies, purchase of equipment and drugs, training of personnel, and introduction of the program in practical public health promoted its successful realization.

[Experience in the elaboration of a program of public health development with medical insurance for an industrial region].

Requirements in medical care and availability of such care for the population of an industrial regions with the towns of Staryĭ Oskol and Gubkin is analyzed. The authors emphasize the unique type of the region. Approaches to improvement of the level of public health in this region at the expense of coordination of the activities of medical and public health institutions, equipment of therapeutic and prophylactic institutions with modern therapeutic and diagnostic devices, and upgrading the knowledge and skills of physicians and paramedical personnel, which may be implemented by professors and teachers of the N.

[The surgical treatment of choledocholithiasis].

In 241 patient choledocholithiasis the transduodenal sphincteroplasty (TDSP) and in 118--choledochoduodenostomy (CDS) was conducted. The incidence of specific postoperative complications after TDSP was 10.3%, after CDS--10.

[Biomechanical evaluation of the effectiveness of surgical treatment of patients with injuries of the bursal and ligamentous apparatus of the knee joint].

Comparative analysis of data of biomechanical examination of statics and walking of 26 patients with fresh and 20 with inveterate damages of bursal and ligamentous apparatus of the knee joint indicated that transformation of the fresh damages in the inveterate ones inevitably leads to formation of compensatory-adaptable mechanisms in locomotor apparatus directed to compensation of disturbed functions. Early operative treatment of fresh damages of bursal and ligamentous apparatus of the knee joint is a highly effective method that is confirmed by normalization of rhythm of movement at all parts of podographic relief bench in 92,5% patients. After plastic operations on ligamentous structures in case of inveterate damages the rhythm of walking is normalized only in 80,7% cases.

[Clinical results of transduodenal sphincteroplasty and choledochoduodenostomy in acute cholecystitis].

In acute cholecystitis of the 460 patients, in 126, besides the cholecystectomy, the transduodenal sphincteroplasty (79) and supraduodenal choledochoduodenostomy (47) were performed. The postoperative lethality was 6.3 and 4.

[Surgical treatment of chronic constipation].

The results of clinical use of periarterial neurectomy of the superior mesenteric artery, the Bauhin's valve plasty according to Vitebsky, and their combination with different variants of colonic resection, subtotal and total colectomy with creation of ileosigmoid and ileorectal anastomosis in 64 patients are presented. The best functional results were noted after subtotal colectomy with the ileosigmoid anastomosis formation.

[Transduodenal sphincteroplasty].

Transduodenal sphincteroplasty was carried out in 237 (24%) of 1002 patients subjected to planned and emergency primary operations for benign lesions of the bile tracts and in 50 (58%) of 86 patients who were operated on for a second time. On the whole, 287 operations for sphincteroplasty accounted for 63% of operations on the hepaticocholedochus. The indications for transduodenal sphincteroplasty (TDSP), its techniques, specific complications, and mortality are discussed on the basis of retrospective analysis of the clinical material.
