Objective: Responsive Neurostimulation (RNS) is a closed-loop neuromodulation therapy approved for treating drug resistant epilepsy (DRE) with 1 or 2 seizure foci, but its potential utility for treating complex seizure networks, such as in focal cortical dysplasia (FCD), remains uncertain. This review and commentary discuss the current practice of RNS use in focal cortical dysplasia-related drug-resistant epilepsy(FCD-DRE), and the potential of individualized approaches.

Methods: Our scoping review followed a search to identify relevant studies on epilepsy and RNS across MEDLINE, Embase, and Web of Science, yielding 674, 1,255, and 579 results, respectively followed by abstract and full text review to include FCD-DRE.

Anthropogenic changes to the environment have been associated with changes in body size of multiple organisms. However, although climate and land use influences on body size have been examined separately, the simultaneous effects and potential interactions of these two factors on body size have rarely been studied. We examined the size and mass of a common North American toad (Fowler's toad, ) using museum specimens from 1931 to 1998 to quantify the potential interactive effects of climate change (temperature and precipitation) and land use change (forested area) on body size.

Humans emerged across Africa shortly before 300 thousand years ago (ka). Although this pan-African evolutionary process implicates diverse environments in the human story, the role of tropical forests remains poorly understood. Here we report a clear association between late Middle Pleistocene material culture and a wet tropical forest in southern Côte d'Ivoire, a region of present-day rainforest.

Purpose: To compare the surgical and microbiological characteristics of fracture related infection (FRI) that had union and those requiring additional surgery to promote bone healing. We hypothesized that FRIs with MRSA will have higher risk for reoperation to promote bone healing.

Methods: This is a retrospective study on 247 patients over 18 who underwent bone fixation for pelvis, upper, and lower extremities fractures between 2013 and 2021 at a level I trauma center.

In human medicine, the choice of medical terminology influences patients' choice of management options and associated anxiety levels in relation to their diagnoses. The objective of this study was to determine the association between canine caregiver's treatment choices and anxiety levels when papillary thyroid cancer is described with or without the term cancer. This randomised cross-sectional study surveyed 683 people over 18 years old over 8.

Background: Internationally, one in three children develop delirium during their intensive care stay. International guidelines strongly recommend twice-daily screening for paediatric delirium using validated instruments. In the United Kingdom and Ireland, delirium was assessed only when suspected and few intensive care units (ICUs) used validated instruments.

Background: There is an increased risk of human papillomavirus (HPV)-associated infections and malignancies among people living with HIV (PLHIV). However, there is limited literature exploring the intersection of HPV, HIV, and oral health in Tanzania and across sub-Saharan Africa. We reviewed the existing literature on the intersection of HIV, HPV, and oral health in Tanzania.

Introduction: Self-harm is a major public health issue in the imprisoned population. Limited high-quality evidence exists for the potential impact of prison environmental factors such as solitary confinement. This exploratory pilot prospective cohort study in a large male remand prison in England sought to estimate effect sizes for a comprehensive range of prison environmental factors in relation to self-harming behaviours.

Objectives: This systematic review examined the literature regarding management of fracture patients who take direct oral anticoagulant (DOAC) medications, with a focus on delay in surgical treatment, and need for transfusions. In addition, a survey of orthopaedic trauma surgeons was conducted to gain insight on current practices.

Data Sources: A review of PubMed, Cochrane, Embase, and Scopus databases was performed from inception through March 2024, including English language publications.

Creutzfeldt-Jakob Disease (CJD) is an exceedingly rare condition with an incidence of one case per million people worldwide. It is diagnosed with symptoms of rapid neurological decline, positive CSF biomarkers, and diagnostic findings on MRI and EEG. There is no known effective therapy for CJD and prompt diagnosis is required to prepare for the expected disease prognosis and goals of care discussions.

Background: Prior studies have proposed defibrillator biosignal algorithms which characterize cardiac arrest rhythm and physiologic status. We evaluated whether a novel, individualized resuscitation strategy that integrates multiple ECG and impedance-based algorithms could reduce CPR interruptions and better align rescuer actions with patient-specific physiology.

Methods: In a retrospective cohort of ventricular fibrillation out-of-hospital cardiac arrests, observed rescuer actions (rhythm analysis, shock delivery, pulse checks, and drug therapy) were compared to hypothetical actions recommended by the proposed individualized strategy.

Objective: The Sedation and Weaning in Children (SANDWICH) trial of a sedation weaning and ventilator liberation bundle had a primary outcome of time to successful extubation, and showed significant but small difference. We explored the impact of the intervention on infants with bronchiolitis.

Design: Post hoc subgroup analysis of a cluster-randomized trial, 2018 to 2019 (ISRCTN16998143).

Background: Although racial disparities have been described in resuscitation, little is known about potential bias in race classification of out-of-hospital cardiac arrest (OHCA).

Methods: We conducted a retrospective cohort study of adults treated by emergency medical services (EMS) for nontraumatic OHCA in King County, WA between January 1, 2018, and December 31, 2021. We assessed agreement using κ and evaluated patterns of missingness between EMS-assessed race versus comprehensive race classification from hospital and death records.

Youth living in low- and middle-income countries (LMICs) have an increased vulnerability to mental illnesses, with many lacking access to adequate treatment. There has been a growing body of interventions using task sharing with trained peer leaders to address this mental health gap. This scoping review examines the characteristics, effectiveness, components of peer delivery and challenges of peer-led mental health interventions for youth aged 10-24 in LMICs.

Introduction: Increased aggression in a regional Comprehensive Psychiatric Emergency Program (CPEP) led to a significant rise in physical assaults, restraints, and use of security personnel. Root cause analysis revealed a need for more extensive training on de-escalation, teamwork and communication.

Aims: This quality improvement project evaluated the impact of an interprofessional, high-fidelity simulation project on interdisciplinary collaboration to manage de-escalation and aggression safely and effectively.

Background: Understanding the degree to which patients are actively involved, confident and capable of engaging with self-management and rehabilitation could be an initial step in guiding individualised supportive strategies for people after critical illness.

Aims: To assess the levels of active involvement with self management among ICU survivors using the Patient Activation Measure (PAM), explore associations between patient characteristics and PAM results, and investigate its relationship with patients' support needs at key transition points during the recovery process.

Methods: Eligible participants received both the PAM and Support Needs After Critical care (SNAC) questionnaires by post.

Purpose: In the adult literature, allograft reconstruction of gapped peripheral nerve injuries has gained popularity over autologous nerve grafting. Allografts have demonstrated similar recovery while eliminating donor site morbidity. There is no well-defined incidence or treatment of such injuries in children.

Efforts to reduce the frequency, extent, and toxicity of harmful algal blooms (HABs) require knowledge about drivers of algal growth, toxin production, and shifts in phytoplankton community composition to cyanobacterial dominance. Although labile nitrogen (N) and phosphorus (P) fuel primary production, micronutrients also play roles as the enzymatic engines that facilitate rapid and efficient growth and toxin production. Macro- and micronutrient availability can shape community composition and function by selecting for particular taxa.

Background: In Europe, approximately 291,000 cardiac arrests occur annually. Despite critical care therapy, hospital mortality remains high. This systematic review assessed whether, in comatose survivors of cardiac arrest, any drug therapy, compared to placebo or usual care, improves outcomes.
