Objective: Improving dementia care is a policy priority nationally and internationally; there is a 'diagnosis gap' with less than half of the cases of dementia ever diagnosed. The English Health Department's Quality and Outcomes Framework (QOF) encourages primary care recognition and recording of dementia. The codes for dementia are complex with the possibility of underidentification through miscoding.

A two-month-old child having WPW syndrome and orthodromic tachycardia was on treatment with digoxin, flecainide and amiodarone. Despite this, he continued to have severe, very frequent episodes of tachycardia. The left-sided accessory pathway was hence ablated via a patent foramen ovale.

A 39 year old married heterosexual male presented with extensive purpuric macules, papules, and lichenoid plaques scattered over his skin and mucocutaneous surfaces for one year. The lesions were larger in both his groins, and were associated with lymphedema of his penis and scrotum. His serology for the HIV-1 virus was positive.

We describe a 65-year-old man who had had a permanent cardiac pacemaker implanted in 1994 and presented now with ascites. He was investigated for 3 months without finding any cause. He was admitted with a colonic bleed due to a polyp; on cardiac evaluation he was found to have pacemaker syndrome.

An 80-year-old woman presented with almost daily episodes of tachycardia. She was found to have drug refractory atrioventricular nodal re-entry tachycardia. As she was found to have bilateral common iliac vein occlusion, radiofrequency ablation of the slow pathway was performed via the subclavian vein.
