A randomized controlled trial examined the effects of non-stress antepartum cardiotocography on obstetric management and assessed its usefulness as a diagnostic test of fetal compromise. Daily cardiotocograph recordings were made in 396 antenatal patients at increased risk of fetal compromise but were withheld from the clinicians responsible for care in half the cases. The frequency of intrapartum fetal distress and low Apgar score was similar in the two groups.

A progestogen (norethisterone) and a dopamine antagonist (sulpiride) were given alone and in combination to volunteers to examine their effects on excretion of ovarian steroids. Compared with non-treatment cycles (n = 15), contraception with a progestogen alone (n = 10) was associated with increased excretion of oestrone and partial suppression of excretion of pregnanediol, suggesting partial luteinization of unruptured follicles. By contrast, the combination of norethisterone and sulpiride (n = 9) suppressed both ovarian steroids to basal values, the suppression being even greater than with sulpiride alone (n = 5).

View Article and Find Full Text PDFWe measured left ventricular chamber dimension and wall thickness using M-mode echocardiography in 61 adolescents with systolic or diastolic blood pressures above the 90th percentile for age and sex and in 49 normotensive adolescents. Left ventricular posterior wall and ventricular septal thickness indexed to body surface area were significantly greater (p less than 0.001) in the hypertensive group than in the normotensive controls.
View Article and Find Full Text PDFProstaglandin E1 infusion and a microporous expanded polytetrafluoroethylene graft were used in the management of eight infants, all less than 4 days old, with interruption of the aortic arch. Five of the six infants receiving prostaglandin E1 responded dramatically to this therapy, with return of lower limb pulses and lessening of metabolic acidosis. There were no adverse effects attributable to the prostaglandin E1 infusion.
View Article and Find Full Text PDFThe relative size of the main pulmonary artery was determined from cineangiograms of 117 patients with d-transposition of the great arteries by calculating the ration between the diameters of the main pulmonary artery and aorta. The pulmonary artery was largest in patients with ventricular septal defect or patent ductus arteriosus, or both, because of increased pulmonary arterial pressure and flow. In patients with an intact ventricular septum or with left ventricular outflow tract obstruction, or both, the main pulmonary artery was approximately the size of the aorta.
