A quasi-experimental study of trigger films for teaching the doctor-patient relationship.

Background: It is challenging to teach the complexity of the doctor-patient relationship through attitude, ethics, and communication (AETCOM) modules, particularly without being formally trained and especially to first-year medical students who do not interact directly with patients. The present study was undertaken to assess the effectiveness of trigger films (TFs) or short movie clips as a teaching-learning tool to train undergraduate medical students on various aspects of doctor-patient relationships.

Methods: Two modules on various aspects of the doctor-patient relationship were developed using TFs and written case studies and implemented on Phase Ⅰ medical students.

As global populations age, the incidence of osteoporotic fractures rises, leading to increased delayed healing and non-union cases. The pathophysiology underlying non-union in osteoporotic patients involves impaired bone regeneration, reduced osteoblast function, and poor vascularity.

Challenges with mobility devices for female Veterans with spinal cord injuries.

Context: The growing number of females entering the armed forces has led to an increase in the number of female Veterans with spinal cord injury and diseases (SCI/D) requiring mobility devices. Limited research exists that explores whether mobility devices meet their needs in terms of comfort, fit and design.

Objective: To characterize respondents with SCI/D who use mobility devices and determine if these devices are meeting their daily needs.

Pashto Handwritten Invariant Character Trajectory Prediction Using a Customized Deep Learning Technique.

Before the 19th century, all communication and official records relied on handwritten documents, cherished as valuable artefacts by different ethnic groups. While significant efforts have been made to automate the transcription of major languages like English, French, Arabic, and Chinese, there has been less research on regional and minor languages, despite their importance from geographical and historical perspectives. This research focuses on detecting and recognizing Pashto handwritten characters and ligatures, which is essential for preserving this regional cursive language in Pakistan and its status as the national language of Afghanistan.

Giant Redox Entropy in the Intercalation vs Surface Chemistry of Nanocrystal Frameworks with Confined Pores.

Redox intercalation involves coupled ion-electron motion within host materials, finding extensive application in energy storage, electrocatalysis, sensing, and optoelectronics. Monodisperse MOF nanocrystals, compared to their bulk phases, exhibit accelerated mass transport kinetics that promote redox intercalation inside nanoconfined pores. However, nanosizing MOFs significantly increases their external surface-to-volume ratios, making the intercalation redox chemistry into MOF nanocrystals difficult to understand due to the challenge of differentiating redox sites at the exterior of MOF particles from the internal nanoconfined pores.

Decoding degeneration: the implementation of machine learning for clinical detection of neurodegenerative disorders.

Machine learning represents a growing subfield of artificial intelligence with much promise in the diagnosis, treatment, and tracking of complex conditions, including neurodegenerative disorders such as Alzheimer's and Parkinson's diseases. While no definitive methods of diagnosis or treatment exist for either disease, researchers have implemented machine learning algorithms with neuroimaging and motion-tracking technology to analyze pathologically relevant symptoms and biomarkers. Deep learning algorithms such as neural networks and complex combined architectures have proven capable of tracking disease-linked changes in brain structure and physiology as well as patient motor and cognitive symptoms and responses to treatment.

A retrospective analysis of the impact of Michigan's opioid prescribing legislation on discharge opioid prescribing at a single institution.

Unlabelled: This study sought to determine if there were any changes in opioid prescribing habits of providers at a single institution after the implementation of legislation to increase opioid prescribing regulations. Our study demonstrated a 39.5 percent decrease in overall morphine milligram equivalent (MME) prescribed the year after the laws took effect when compared with the year prior.

Trigger films to teach core competencies of ethics and professionalism to first-year medical and nursing students.

Professionalism and communication were formally introduced in India's Competency-Based Curriculum (CBC) as part of the five roles of an Indian medical graduate and 10 core competencies in the Bachelor of Science Nursing program. It may be challenging to teach the complexity of clinical medical ethics to undergraduate students at a young age in the absence of direct patient interaction. Trigger films (TFs) are brief (3-10 min) clips that have been used in the West to provoke debate, promote reflection, and assist trainees in dealing with ethical dilemmas.

Diagnostic Value Of Serum Urea, Creatinine, Sodium And Potassium For Complicated Appendicitis - A One Year Retrospective Study.

Background: Acute appendicitis is a common surgical emergency and if complicated, manifesting as abscess, perforation, suppurative or gangrenous appendicitis, carries significant morbidity and mortality. Its early preoperative diagnosis can improve outcome in patients and reduce incidence of negative appendectomies. It, however, mandates identification of new diagnostic parameters for it.

COVID-19 Presentation in an Acute Motor Incomplete Spinal Cord Injury in Inpatient Rehabilitation.

Spinal cord injuries lead to impairment of the central regulation of respiratory muscle activity. This impairs the cough response, which can increase the risk of complications if infected with coronavirus disease 2019. This case describes a 32-yr-old man with an acute traumatic motor incomplete spinal cord injury, C4 American Spinal cord Injury Association Impairment Scale D D, in an inpatient rehabilitation facility who presented with only a fever.

Paradoxical reaction to increased doses of intrathecal baclofen in a patient with Leigh syndrome.

In clinical practice, intrathecal baclofen (ITB) therapy is used to control spasticity. After initial placement of the ITB pump, clinicians incrementally increase the dose until effectiveness in alleviating spasms and spasticity is optimized. However, this case describes a 4-year-old male with Leigh syndrome who developed a paradoxical worsening of spasticity and pain with incremental increase of his ITB pump.

Poor patient awareness and frequent misdiagnosis of migraine: findings from a large transcontinental cohort.

Background And Purpose: Although migraine is the second most disabling condition worldwide, there is poor awareness of it. The objective was to assess the awareness of migraine and previous diagnostic and therapeutic consultations and treatments in a large international population of migraineurs.

Methods: This was a multicentre study conducted in 12 headache centres in seven countries.

Prolonged migraine aura: new insights from a prospective diary-aided study.

Background: There is limited literature on prolonged aura (PA - defined as an aura including at least one symptom for > 1 h and < 7d), and there are no prospective studies. The aim of this study is to characterize prospectively the phenotype and prevalence of PA.

Findings: Two hundred and twenty-four patients suffering from migraine with aura were recruited from the Headache Centers of Pavia and Trondheim.

Development and validation of simulated virtual patients to impart early clinical exposure in endocrine physiology.

In the country presently, preclinical medical students are not routinely exposed to real patients. Thus, when they start clinical postings, they are found to have poor clinical reasoning skills. Simulated virtual patients (SVPs) can improve clinical skills without endangering real patients.

Multi-organ failure, weeks after being exposed to murky water - rule out leptospirosis.

Leptospirosis is a rare zoonotic disease which occurs in people exposed to contaminated water or/and animal urine. We report two cases of moderate to severe leptospirosis. The first case was a healthy middle-aged biology professor who recently visited his family in Jamaica and presented with multi-organ failure.

Evaluation of the Auditory Pathway in Traffic Policemen.

Background: Traffic policemen working at heavy traffic junctions are continuously exposed to high level of noise and its health consequences.

Objective: To assess the hearing pathway in traffic policemen by means of brainstem evoked response audiometry (BERA), mid-latency response (MLR), and slow vertex response (SVR).

Methods: In this observational comparative study, BERA, MLR, and SVR were tested in 35 male traffic policemen with field posting of more than 3 years.

In vitro enzymatic investigation of Ficus carica (Fruit).

The present study demonstrates the biological study of Ficus carica fruit. Methanolic extract of plant fruit was prepared and evaporated under reduced pressure by rota vapor and n- hexane, Chloroform, ethyl acetate and n-butanol soluble fractions were prepared separately from crude methanolic extract. These fractions were then screened for acetyl cholinesterase, butryl cholinesterase and lipoxygenase activities.

A further aim is to assess its association with vitamin B12 deficiency induced by metformin. Thirty diabetics taking metformin and 30 age-matched non-diabetic controls were enrolled.
