Regional anaesthesia has seen a resurgence of sorts since the widespread advent of ultrasound into clinical practice. The ability to access hitherto inaccessible nerves and fascial planes in the human body whilst ensuring visualisation of the needle tip during block performance has opened the proverbial floodgates leading to its widespread adoption, further supported by a growing body of evidence for its many benefits in a patient's perioperative journey and pain management. The concomitant advancement of technology and the development of powerful simulation and artificial intelligence tools has given a much-needed impetus towards improving training and safe practice in regional anaesthesia.

Article Synopsis
  • Appendiceal mucinous neoplasms, which are rare and make up less than 1% of all cancers, are becoming more common and pose significant treatment challenges, particularly due to their tendency to spread within the abdominal cavity.
  • A 42-year-old man from Chad experienced severe abdominal pain and was diagnosed with pseudomyxoma peritonei, likely originating from an appendiceal tumor; he underwent aggressive management including chemotherapy and hyperthermic intraperitoneal chemotherapy (HIPEC).
  • The patient's treatment involved removing affected organs, and the prognosis largely depends on the tumor's grade and the degree of invasion.

Background: The diagnostic performance of the available risk assessment models for VTE in patients who are critically ill receiving pharmacologic thromboprophylaxis is unclear.

Research Question: For patients who are critically ill receiving pharmacologic thromboprophylaxis, do risk assessment models predict who would develop VTE or who could benefit from adjunctive pneumatic compression for thromboprophylaxis?

Study Design And Methods: In this post hoc analysis of the Pneumatic Compression for Preventing VTE (PREVENT) trial, different risk assessment models for VTE (ICU-VTE, Kucher, Intermountain, Caprini, Padua, and International Medical Prevention Registry on VTE [IMPROVE] models) were evaluated. Receiver-operating characteristic curves were constructed, and the sensitivity, specificity, positive and negative predictive values, and positive and negative likelihood ratios were calculated.

The pollution of heavy metals (HMs) is a major environmental concern for agricultural farming communities due to water scarcity, which forces farmers to use wastewater for irrigation purposes in Pakistan. Vegetables grown around the cities are irrigated with domestic and industrial wastewater from areas near mining, paint, and ceramic industries that pollute edible parts of crops with various HMs. Cadmium (Cd) is an extremely toxic metal in arable soil that enters the food chain and damages the native biota, ultimately causing a reduction in plant growth and development.

Schizophrenia, a global mental health disorder affecting approximately 1 % of the population, is characterized by neurotransmitter dysregulation, particularly dopamine, serotonin, and glutamate. Current antipsychotic therapies, despite their efficacy, are accompanied by adverse effects, which has motivated researchers to investigate more secure substitutes. This study examines the potential antipsychotic effects of esculetin, a natural coumarin derivative recognized for its wide-ranging pharmacological activities (anti-inflammatory, antioxidant, anti-pathogenic, anticancer, and neuroprotective), in animal model of schizophrenia induced by ketamine.

Open pancreaticoduodenectomy, also known as Whipple surgery, is a complex and painful procedure that requires a multi-modal analgesic approach for successful post-operative rehabilitation and recovery. While thoracic epidural analgesia (TEA) remains the gold standard for pain relief after open upper abdominal surgery, it carries many risks that may outweigh the potential benefits of the technique. Furthermore, in laparoscopic converted to open pancreaticoduodenectomy cases, post-operative placement of a thoracic epidural catheter is inconvenient to the patient due to pain and positioning.
