Introduction: Mature teratomas, a diverse group of germ cell tumors, rarely present with complications such as rupture.

Case Presentation: We present a case of a 27-year-old male who was admitted to our hospital with acute dyspnea. A tamponade was diagnosed and the patient underwent a pericardial puncture.

Perineal hernias occur rarely following abdominoperineal resections. No standardized surgical approach exists for treating PH. We herein present the case of a large, symptomatic PH that was repaired with a combined abdominal and perineal approach, with peritoneal flap reconstruction of the pelvic floor and placement of a biological mesh.

Article Synopsis
  • Relapse after stopping immunosuppression treatment is common in Autoimmune Hepatitis (AIH) patients, and there's a need for better non-invasive biomarkers to predict this risk.
  • The study analyzed blood samples from 58 active AIH patients, 56 in remission, and 31 with NASH, focusing on the levels of activated T cell subsets and the cytokine BAFF.
  • Results showed that increased frequencies of certain activated T cells and high BAFF levels in remission patients were strongly linked to a higher chance of relapse after treatment withdrawal, suggesting these could serve as useful biomarkers for monitoring AIH.

Article Synopsis
  • Acute diverticulitis is a common colorectal emergency in the Western world, and its management has evolved significantly over time.
  • Recent evidence emphasizes the importance of the microbiome and genetic factors in understanding the disease, while traditional antibiotic use for uncomplicated cases may no longer be necessary.
  • There are shifts in the recommendations for follow-up procedures, like colonoscopy, which now only applies to specific patient groups, though there are still gaps in understanding certain forms of the disease.

Background: Caring for patients with advanced cancer is complex and challenging, requiring varied expertise, including symptom management, communication skills, care coordination and emotional resilience. Within existing literature, the lived experiences of oncology nurses are poorly articulated in countries with a lower income where formal palliative care (PC) is absent.

Aim: To explore the lived experiences of Gazan oncology nurses who provide care to patients with advanced cancer in healthcare systems, without formal palliative care infrastructure.

Background & Aims: Utility, a major principle for allocation in the context of transplantation, is questioned in patients with acute-on-chronic liver failure grade 3 (ACLF-3) who undergo liver transplantation (LT). We aimed to explore long-term outcomes of patients included in a three-centre retrospective French study published in 2017.

Method: All patients with ACLF-3 (n = 73), as well as their transplanted matched controls with ACLF-2 (n = 145), 1 (n = 119) and no ACLF (n = 292), who participated in the Princeps study published in 2017 were included.

  • * A case study details a young patient with heart issues identified as restrictive heart disease alongside primary amenorrhea, leading to her Kallmann syndrome diagnosis.
  • * The rare combination of cardiac problems in this syndrome suggests there might be an underlying genetic link that warrants further investigation.

Objectives: This study aimed to review the blood stream infections of major burn patients in a tertiary care burn unit to determine the most prevalent infecting organisms in order to have a better empirical therapy protocol.

Methods: This retrospective study analysed the blood stream infection of 155 major burn (>20% Total Body Surface Area [TBSA]) patients in Khoula Hospital, Muscat, Oman between January 2014 to December 2019.

Results: The median age was 33 years and 57.

Background And Objectives: EUS is a potential alternative for the drainage of abscesses. The aim of this study was to determine if EUS-guided pelvic abscess drainage is technically feasible, safe, and a valid option for abscess resolution.

Methods: We conducted a retrospective review from 2002 to 2020 at a single quaternary institution.

NK cells are innate immune cells that target infected and tumor cells. Mature NK (mNK) cells undergo functional maturation characterized by four distinct stages, during which they acquire their cytotoxic properties. mNK cells from non-obese diabetic (NOD) mice exhibit a defect in functional maturation and have impaired cytotoxic functions.

Introduction: Peritoneal mesothelioma (PM) is a rare malignancy originating from the peritoneal lining. Cytoreductive surgery (CRS) and heated intraperitoneal chemotherapy (HIPEC) is the standard-of-care for patients with isolated PM. Due to a paucity of prospective data there are several different HIPEC protocols.

Article Synopsis
  • Klatskin tumor, or hilar cholangiocarcinoma, was highlighted in a case report using two different imaging techniques: 68 Ga-FAPI and 18 F-FDG PET/CT.
  • The case showcased that the 18 F-FDG PET/CT produced a false-negative result for the main tumor site, indicating a limitation of that imaging method.
  • In contrast, 68 Ga-FAPI showed positive uptake at the same location, demonstrating its effectiveness in identifying the tumor.

Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy (CRS-HIPEC) is a treatment option for peritoneal metastases (PM) but is associated with significant postoperative morbidity. The aim of this study was to determine the prognostic value of computed tomographic (CT)-measured sarcopenia on postoperative outcomes and survival in patients undergoing CRS-HIPEC for PM from various origins. A retrospective cohort study was conducted between 2012 and 2020.

Introduction: Severe alcohol-related hepatitis (AH) is associated with an increased risk of infection, but the impact of pneumonia has not been specifically analyzed in a specific cohort.

Methods: All patients admitted for severe AH between 2002 and 2020 were prospectively included. Systematic screening for infection was performed at admission and renewed in the case of clinical suspicion.

This article describes progress relating to a previously reported matched filter retrieval approach for the estimation of hurricane maximum winds using delay Doppler map (DDM) measurements of the Cyclone Global Navigation Satellite System (CYGNSS) mission. The retrievals presented are based on comparisons of these measured DDMs, and their simulated counterparts as a set of storm parameters are varied. The analysis presented examines the dependencies of retrieval performance on the synthetic storm model used as part of the forward modeling process using a set of CYGNSS storm-observing full DDM downlinks containing 68 tracks and spanning 18 storms over the period 2017-2020.

Article Synopsis
  • There is ongoing debate on the best methods to lower central venous pressure during liver surgery, which can help reduce blood loss and the need for blood transfusions.
  • A study analyzed data from cancer patients undergoing hepatectomy between 2011 and 2017 to evaluate the impact of intraoperative hypovolemic phlebotomy (IOHP) on blood transfusion needs and surgical outcomes.
  • Results showed that patients who received IOHP had a lower risk of needing transfusions on the day of surgery compared to those who did not (6.9% vs. 12.4%), although the overall incidence of severe postoperative complications was similar between the two groups.

The use of temporary hip prosthesis made of orthopedic cement (spacer) in conjunction with antibiotics became a widespread method used for treating prosthetic infections despite the fact that this method makes bone cement (PMMA) more fragile. The necessity to incorporate reinforcement is therefore crucial to strengthen the bone cement. In this study, a validated Finite Element Modelling (FEM) was used to analyze the behavior of spacers.

Background And Aims: Because of the extensive use of this drug, further evaluation of acute liver injury (ALI) with therapeutic doses of acetaminophen (APAP; ≤6 g/d) is required. We characterize ALI with therapeutic doses of APAP and determine the host factors associated with disease severity and the predictors of outcome.

Approach And Results: All patients admitted with severe APAP-related ALI in our center were included from 2002 to 2019, either attributable to therapeutic doses or overdose.

Isolated bilateral periorbital edema with negative serology is an extremely rare presentation of cutaneous lupus erythematosus that may lead to eyelid scarring, infection, or even corneal involvement. The treatment usually comprises a combination of hydroxychloroquine and a tapering dose of systemic steroids. Patients require long-term follow-up as they may develop systemic lupus erythematosus with positive serology later in life.
