Video 1Selective biliary cannulation was difficult, and the catheter tended to be cannulated into the pancreatic duct. We applied a double-wire technique to achieve biliary cannulation. Finally, selective biliary cannulation was achieved.

Article Synopsis
  • The study compared the effectiveness of texture and color enhancement imaging (TXI) versus white light imaging (WLI) for biliary cannulation in patients who had Roux-en-Y gastrectomy.
  • TXI showed a higher success rate (93.9% vs. 83.7%) and significantly reduced the median time to successful cannulation (10 min vs. 18 min) compared to WLI.
  • Both imaging techniques had similar adverse event rates, indicating that TXI is a safe option for this procedure.

Background/aims: Endoscopic ultrasound-guided tissue acquisition (EUS-TA) using Franseen needles is reportedly useful for its high diagnostic yield. This study compared the diagnostic yield and puncturing ability of EUS-TA using 22-gauge cobalt-chromium (CO-Cr) needles with those of stainless-steel Franseen needles in patients with solid pancreatic lesions.

Methods: Outcomes were compared between the 22-gauge Co-Cr Franseen needle (December 2019 to November 2020; group C) and stainless-steel needle (November 2020 to May 2022; group S).

Article Synopsis
  • Upon examination of the papilla, fresh blood was noted, but the source of bleeding wasn't visible with standard white-light imaging.
  • We transitioned to texture- and color-enhancement imaging, which helped us locate a visible vessel as the source of the bleed.
  • Successful endoscopic hemostasis was performed using a hemostatic clip to stop the bleeding.

Article Synopsis
  • Scirrhous gastric cancer (SGC) is difficult to diagnose through standard methods like endoscopy and biopsy due to its unique growth patterns, leading researchers to explore endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) as a potential alternative.
  • A systematic review analyzed the use of EUS-FNA on 114 patients suspected of having SGC, finding an overall diagnostic success rate of 82.6% with no reported adverse events.
  • The study indicates that EUS-FNA could be a valuable option for diagnosing SGC, especially for patients who have inconclusive results from traditional biopsy methods.

Background: Myalgic encephalomyelitis/chronic fatigue syndrome/systemic exertion intolerance disease (ME/CFS/SEID) is a condition diagnosed primarily based on clinical symptoms, including prolonged fatigue and post-exertional malaise; however, there is no specific test for the disease. Additionally, diagnosis can be challenging since healthcare professionals may lack sufficient knowledge about the disease. Prior studies have shown that patients with ME/CFS/SEID have low serum acylcarnitine levels, which may serve as a surrogate test for patients suspected of having this disease.

Endoscopic retrograde cholangiopancreatography (ERCP) in patients with surgically altered anatomy is technically challenging. For example, scope insertion, selective cannulation, and intended procedures, such as stone extraction or stent placement, can be difficult. Single-balloon enteroscopy (SBE)-assisted ERCP has been used to effectively and safely address these technical issues in clinical practice.

A 76-year-old man diagnosed with early-stage colorectal cancer was referred to our hospital for endoscopic submucosal dissection (ESD). The patient had a low platelet count (31,000/µL) due to immune thrombocytopenia (ITP). The cancerous lesion was completely resected without any adverse events.

We herein report a case of intraperitoneal abscess as a postoperative complication of gastric endoscopic submucosal dissection (ESD). A 70-year-old man who underwent ESD for early gastric cancer sought consultation for abdominal pain on postoperative day 28. Abdominal computed tomography revealed intraperitoneal abscess rupture.

Anaplasma phagocytophilum, an obligate intracellular bacterium that propagates within host granulocytes, is considered to modify the host intracellular environment for pathogenesis. However, the mechanism(s) underlying such host modifications remain unclear. Here, we aimed to investigate the relation between A.
