Objectives: The endoscopic channel can be damaged by instruments during use and cleaning, leading to contamination, infection, and increased repair costs. However, few devices are available to observe the inside of the endoscopic channel. This study employed an ultrafine-diameter scope to examine damage in the endoscopic channel.

Background And Aim: The use of a hood at the tip of a colonoscope enables aspiration, inversion of the diverticulum, and observation of the inside of the diverticulum. In most previous studies, a short hood was used; however, observation of the diverticulum is often inadequate. Long food is promising by previous research, but it was a retrospective study using propensity matching and has some limitations.

Background: Adenoma detection rate (ADR) is an important indicator of colonoscopy quality and colorectal cancer incidence. Both linked-color imaging (LCI) with artificial intelligence (LCA) and LCI alone increase adenoma detection during colonoscopy, although it remains unclear whether one modality is superior. This study compared ADR between LCA and LCI alone, including according to endoscopists' experience (experts and trainees) and polyp size.

Revised idiopathic pulmonary fibrosis treatment guidelines were published in 2015, and nintedanib was conditionally recommended. Although diarrhea is reported to be a common major adverse event associated with nintedanib, there have been few reports on detailed endoscopic findings of nintedanib-associated enterocolitis. A 74-year-old woman was diagnosed with idiopathic pulmonary fibrosis 4 years ago in May.

Pneumatosis cystoides intestinalis is an uncommon disease that rarely causes intussusception. We report the case of a 16-year-old male patient with intermittent abdominal pain who was diagnosed with intussusception. The patient had no history of raw food ingestion, fever, diarrhea, or hematochezia.

The pathologic diagnosis of duodenal tumors is a developing field; however, its overview remains unclear. We describe a rare case of a duodenal gastric-type neoplasm in a 50-year-old woman. She visited her primary care doctor with complaints of upper abdominal pain, tarry stools, and shortness of breath on exertion.

AdvanCE is a useful device in patients who cannot swallow capsule endoscopes, or when these remain in the patient's stomach without passing into the duodenum. A net or snare may be used to guide the capsule endoscope into the duodenum when the capsule endoscope becomes stuck in the stomach, but it may be difficult to guide the capsule into the duodenum in some cases, particularly in those of waterfall stomach. In such cases, AdvanCE is found to be an effective guidance tool.

Zinc intake has reduced hospitalizations in patients with ulcerative colitis (UC), highlighting the need to maintain blood zinc levels. This prospective study investigated whether the promotion of zinc intake and a Japanese diet (high in -3 fatty acids) could induce clinical remission in patients with mild active UC. Patients with mild active UC were randomly assigned to either (1) continue an unrestricted diet or (2) receive nutritional guidance promoting zinc intake and a Japanese diet.

Several new treatments for ulcerative colitis have been developed recently. The depletion of leukocytes by granulocyte and monocyte adsorption apheresis (GMA) was developed and adapted for patients with ulcerative colitis with rare adverse events. We investigated whether treatment with GMA and prednisolone (GMA + PSL) is more effective than PSL alone for patients with moderate to severe ulcerative colitis.

Article Synopsis
  • This study compares the diagnostic accuracy and histological quality of two types of needles (Franseen and Fork-tip) used in endoscopic ultrasound-guided fine-needle biopsy (EUS-FNB) for solid pancreatic lesions.
  • A total of 147 patients were analyzed, with the Fork-tip needle showing 93.3% diagnostic accuracy, comparable to the Franseen needle, without significant differences in tissue quality or operator skill changes.
  • The findings suggest that both needle types are equally effective for EUS-FNB, making the choice between them largely a matter of preference.

Article Synopsis
  • The study focuses on the importance of understanding the structure of the bile duct opening to improve bile duct cannulation success and investigates whether texture and color enhancement imaging (TXI) can help inexperienced trainees recognize the papilla of Vater better than standard white light imaging.
  • Involving 31 patients, trainee endoscopists used both imaging methods to identify the papilla, and their accuracy was compared to that of supervising physicians using statistical analysis.
  • Results showed that TXI significantly improved the congruence in identification among trainees, suggesting its potential as a useful tool in clinical settings to reduce cannulation time and lower the risk of complications like pancreatitis.

Background: There is only one report of Barrett's esophagus (BE) with mixed neuroendocrine-non-neuroendocrine neoplasm (MiNEN). Herein, for the first time, we present a case with an aggressive esophageal MiNEN, as well as with both primary MiNEN and conventional adenocarcinoma, arising in BE.

Case: A 68-year-old woman had been diagnosed with 0-IIa type adenocarcinoma in the background of long-segment BE, 45 months earlier.

Article Synopsis
  • - The study focused on assessing the effectiveness and safety of using short-type single-balloon enteroscopy (SBE) for stone extraction in patients who had a Roux-en-Y gastrectomy, surveying data from September 2011 to January 2022.
  • - Out of 85 patients, the initial procedure saw a 68.2% success rate for complete stone extraction, while the overall success rate (including follow-up procedures) was 87.1%, with an adverse event rate of 8.2%.
  • - Findings revealed that the diameter of the bile duct significantly influenced the success of initial stone extraction, suggesting that larger bile ducts may require multiple sessions and better equipment for optimal results in patients with altered anatomy.

Colorectal endoscopic submucosal dissection (ESD) is a difficult procedure, and its introduction to trainees has been debated. Although the criteria for performing colorectal ESD vary among institutions, it is often allowed after gaining experience performing surgeries in animals and upper gastrointestinal ESD. This pilot study aimed to compare the treatment outcomes of ESD performed by trainees using the multi-loop traction device (MLTD group) and those of conventional ESD performed by experts (control group).

Objectives: This study aimed to evaluate the trainees' practice and learning curve in short-type single-balloon enteroscopy (short SBE)-assisted endoscopic retrograde cholangiopancreatography (ERCP) for patients with surgically altered anatomy (SAA) and determine how to train these trainees.

Methods: The data of short SBE-assisted ERCP procedures between September 2011 and June 2021 were analyzed.

Results: Three trainees and 180 cases were included in the analysis.

Endoscopic ultrasonography (EUS) and endoscopic retrograde cholangiopancreatography (ERCP) for pancreatobiliary diseases require advanced techniques. We usually use an oblique-viewing endoscope in such procedures. Sometimes, tumor invasion causes gastrointestinal strictures.

Background And Aim: This study aimed to compare the mean number of adenomas in patients undergoing Endo-wing-assisted colonoscopy (EAC) and transparent hood-assisted colonoscopy (TAC).

Methods: Patients undergoing colonoscopy for positive fecal immunochemical tests, colon polyp surveillance, and evaluation of abdominal symptoms at a single institution were randomly assigned to the EAC or TAC group. The mean number of adenomas per patient, adenoma detection rate, cecal intubation time, withdrawal time, mean number of adenomas per location, and adenoma size were compared.
