Article Synopsis
  • POEMS syndrome is a rare disorder marked by peripheral neuropathy, high levels of serum vascular endothelial growth factor, and specific monoclonal plasma cell proliferation.
  • Researchers sequenced monoclonal immunoglobulin lambda light chain variable regions in 11 patients with the syndrome, finding that all were restricted to the V lambda 1 subfamily.
  • Two germlines were identified for these patients, showing high average homology, indicating that the restricted use of IGL V lambda 1 germlines may significantly contribute to the disease’s pathogenesis.

Article Synopsis
  • Dexamethasone is commonly used to treat multiple myeloma, but resistance to the drug is a significant clinical challenge.
  • CD44 is identified as an adhesion molecule that helps multiple myeloma cells survive by interacting with hyaluronan, which is linked to increased resistance to Dexamethasone.
  • Engaging CD44 with antibodies or hyaluronan can protect myeloma cells from the growth-inhibiting effects of Dexamethasone, suggesting that targeting CD44 may be a strategy to overcome drug resistance.

Unexpectedly, this episode left him in a prolonged state of unconsciousness, which necessitated neurological testing and imaging.
