Introduction: Complement 3 glomerulopathy (C3G) and primary immune complex membranoproliferative glomerulonephritis (IC-MPGN) have high risks for disease recurrence and allograft loss in transplant kidneys. Pegcetacoplan (targeted complement 3 [C3]/C3b inhibitor) may prevent excessive deposition of C3 and complement 5 [C5] breakdown products and associated renal damage.

Methods: NOBLE (NCT04572854) is a prospective, phase 2, multicenter, open-label, randomized controlled trial evaluating the efficacy and safety of pegcetacoplan in posttransplant patients with recurrent C3G or IC-MPGN.

Experience shapes the brain as neural circuits can be modified by neural stimulation or the lack of it. The molecular mechanisms underlying structural circuit plasticity and how plasticity modifies behaviour are poorly understood. Subjective experience requires dopamine, a neuromodulator that assigns a value to stimuli, and it also controls behaviour, including locomotion, learning, and memory.

Article Synopsis
  • Age-related macular degeneration (AMD) is the primary cause of permanent blindness in developed countries, with certain genetic variations in the complement factor I (CFI) gene increasing the risk of developing the disease.
  • Research focusing on the Finnish population revealed novel CFI rare variants (RVs) in individuals with dry AMD, highlighting the need to understand how these genetic factors influence disease progression.
  • Functional assays showed the G547R variant severely disrupts CFI's regulatory role in the complement system, while another variant, G328R, affects protein production; these findings suggest that certain RVs significantly contribute to AMD risk in this population.

Objective: The aim of this article to outline proposals for the use of high stakes technical performance assessments, defined as assessments of technical skill that take place outside of the operating theatre, across the continuum of surgical training.

Summary Background Data: Technological advancements have allowed for the assessment of operative technical skill outside of the workplace environment, developed and evaluated in line with modern concepts of assessment validity. With the introduction of competency-based education principles across surgical training curricula, increasing scrutiny is being placed on the validity and reliability of assessments used to inform high-stakes training decisions regarding selection, progression through training, autonomy granting and end-of-training certification.

Article Synopsis
  • The term atypical hemolytic uremic syndrome (aHUS) originated in the 1970s to differentiate between familial/sporadic cases and typical epidemic cases associated with Shiga toxin.
  • Over time, aHUS has become a broad term for various diseases that don't relate to Shiga toxin, complicating the definition and treatment strategies due to its diverse causes.
  • A group of experts used a consensus-building method called the Delphi approach to discuss and clarify the terminology and issues surrounding aHUS in light of advancements in medical science and targeted therapies.

Article Synopsis
  • The study compares the effectiveness and safety of eculizumab prophylaxis versus rescue therapy for kidney transplant recipients with atypical hemolytic uremic syndrome (aHUS), analyzing data from Dutch and UK patient cohorts.
  • Results indicated similar graft survival rates between Dutch patients receiving rescue therapy and UK patients on prophylaxis, with Dutch patients showing slightly better outcomes but not significantly different.
  • The findings suggest that in patients with lower genetic risk for recurrence, eculizumab rescue therapy may be as effective as prophylaxis without compromising graft survival.

Background: Surgical handover is associated with a significant risk of care failures. Existing research displays methodological deficiencies and little consensus on the outcomes that should be used to evaluate interventions in this area. This paper reports a protocol to develop a core outcome set (COS) to support standardisation, comparability, and evidence synthesis in future studies of surgical handover between doctors.

Rare variants (RVs) in the gene encoding the regulatory enzyme complement factor I (CFI; FI) that reduce protein function or levels increase age-related macular degeneration risk. A total of 3357 subjects underwent screening in the SCOPE natural history study for geographic atrophy secondary to age-related macular degeneration, including CFI sequencing and serum FI measurement. Eleven CFI RV genotypes that were challenging to categorize as type I (low serum level) or type II (normal serum level, reduced enzymatic function) were characterized in the context of pure FI protein in C3b and C4b fluid phase cleavage assays and a novel bead-based functional assay (BBFA) of C3b cleavage.

Hospital-acquired infections (HAIs) remain a significant factor in hospitals, with implant surfaces often becoming contaminated by highly resistant strains of bacteria. Recent studies have shown that electrical plasma discharges can reduce bacterial load on surfaces, and this approach may help augment traditional antibiotic treatments. To investigate this, a cold atmospheric plasma was used to deposit tobramycin sulphate onto various surfaces, and the bacterial growth rate of in its planktonic and biofilm form was observed to probe the interactions between the plasma discharge and the antibiotic and to determine if there were any synergistic effects on the growth rate.
