Article Synopsis
  • - A 76-year-old woman with advanced gastric cancer showed a remarkable complete response after receiving just 3 courses of nivolumab, following multiple rounds of chemotherapy and surgeries.
  • - After initial treatments led to recurrence, nivolumab was administered as a third-line therapy, but was quickly halted due to kidney issues, even though it resulted in normalized CEA levels and a confirmed complete response.
  • - This case highlights the potential effectiveness of short-term nivolumab in treating unresectable advanced gastric cancer and raises questions about the necessity of continued treatment once a complete response is achieved.

We have introduced neoadjuvant chemotherapy(NAC)for locally advanced lower rectal cancer(LARC)since 2014. We report 2 LARC cases in which a pathological complete response(pCR)was obtained by using cetuximab(Cmab)plus oxaliplatin plus S-1(SOX)for NAC. Case 1: A 68-year-old woman underwent 4 courses of Cmab plus SOX for Rb rectal cancer (well-differentiated adenocarcinoma, cStage Ⅲc: cT4b[vagina]N3M0).

Application of laparoscopic surgery(Lap)for colorectal cancer has expanded, and laparoscopic total pelvic exenteration (TPE)of locally advanced rectal cancer(LARC)invading the urogenital organs has been introduced in some institutions. In our institute, we have performed Lap TPE and posterior-TPE(PPE)in a total of 6 LARC patients so far. Here, we report the surgical technique of Lap TPE, and the associated surgical and short-term outcomes.

Article Synopsis
  • A 70-year-old man with anemia was diagnosed with advanced rectal cancer after a colonoscopy and CT scan, prompting the initiation of neoadjuvant chemotherapy.
  • Following chemotherapy, he underwent a laparoscopic pelvic surgery, but experienced significant postoperative bleeding due to a severe vitamin K deficiency.
  • The deficiency was attributed to decreased vitamin K intake, disruption of its metabolism caused by antibiotics, and suppression of gut bacteria, highlighting the need for monitoring vitamin K levels in patients after surgery.

Article Synopsis
  • Ischemia-reperfusion (IR) injury in the small intestine is a major concern during surgeries like abdominal aortic aneurysm repairs and small intestine transplants, prompting research into treatment methods like Active Hexose Correlated Compound (AHCC), an anti-inflammatory drug from alternative medicine.
  • In a study involving rats, those supplemented with 2% AHCC showed less intestinal damage and thicker mucosal tissues after ischemic events compared to those without AHCC, indicating a protective effect.
  • AHCC was found to reduce the expression of inflammatory factors and inhibit key signaling pathways related to inflammation, suggesting its potential as a treatment for intestinal IR injury in clinical settings.

Amino acids can exert protective effects on the liver either when administered as a medication or following an operation. In this study, we examined the protective effects of amino acids on the liver using in vitro and in vivo models by studying their influence on the induction of inducible nitric oxide synthase (iNOS) and nitric oxide production as a liver injury marker in cultured hepatocytes and liver-protective effects in d-galactosamine and lipopolysaccharide (GalN/LPS)-treated rats, respectively. Primary cultured rat hepatocytes were treated with interleukin (IL)-1β in the presence or absence of Elental® amino acid component (EleAA; 17 amino acids).
