Factors influencing the decision to offer immediate breast reconstruction after mastectomy for ductal carcinoma in situ (DCIS): the Institut Gustave Roussy Breast Cancer Study Group experience.

Background: The increased rate of ductal carcinoma in situ (DCIS) is associated with a rise in indications for mastectomy and immediate breast reconstruction (IBR). The purpose of our study was to evaluate the factors affecting the indications for IBR and its modalities.

Study Design: Data concerning two hundred and thirty-eight consecutive patients with DCIS who had undergone modified radical mastectomy and a sentinel lymph node biopsy (SLNB) between 2005 and 2011 were extracted from our database.

Axillary Padding without Drainage after Axillary Lymphadenectomy - a Prospective Study of 299 Patients with Early Breast Cancer.

BACKGROUND: After lymphadenectomy for early breast cancer, seroma formation is a constant event requiring a suction drainage. This drainage is the strongest obstacle to reducing the hospital stay. Axillary padding without drainage appears to be a valuable option amid the various solutions for reducing the hospital stay.

[Breast reconstruction: state of the arts].

Radical mastectomy is still required in many cases, such as inflammatory breast cancer, multicentric breast cancer, large tumour volume and small breast size. In this setting, immediate breast reconstruction is more and more offered for breast cancer patients. But such plastic surgery is still debated, owing to risks of implant complications when postoperative radiotherapy of chest wall is mandatory in locoregional breast cancer management.

Modification of flap design for total mobile tongue reconstruction using a sensitive antero-lateral thigh flap.

Reconstruction of large carcinologic tongue defect is mandatory to recover adequate speech and swallowing. Free flaps provide thin and pliable tissues needed to restore the shape and the volume of the tongue but their functional outcomes, especially in case of total mobile tongue reconstruction, are still limited. The authors describe a modification of flap design called the 'cathedral triptych' used with the antero-lateral thigh flap.

Statins: a new pharmacological agent for free flap surgery?

Microvascular free tissue transfer has become the standard for reconstruction in head and neck oncological surgery. Several pharmacological agents have been used in order to increase the success rate of this surgery, but there is currently no consensus for an ideal drug. We review the literature concerning the complications encountered in free flap surgery related to the ischaemia-reperfusion injury and detail the effects of statins relevant to this endothelial dysfunction.

Pseudomonas aeruginosa endophthalmitis caused by contamination of the internal fluid pathways of a phacoemulsifier.

Purpose: To report 4 cases of Pseudomonas aeruginosa endophthalmitis caused by internal contamination of the internal pathways of a phacoemulsifier.

Setting: Ophthalmology Center, Perpignan, France.

Methods: Four clinical cases of postoperative endophthalmitis occurred after phacoemulsification.

[Acute necrotizing retinitis. Apropos of 2 cases].

Two cases of acute retinal necrosis are reported, the etiology of this affection being priorly defined. The lesions were bilateral in the first case, and retinal detachment developed: the first eye was inoperable, and surgery was unsuccessful for the second eye in spite of a salvage vitrectomy after a classical operation. We therefore treated the second case with immediate and massive corticotherapy, and, mainly, preventive total vitrectomy as soon as the first signs of vitreous retraction appeared and before the occurrence of retinal detachment.
