Introduction: Social determinants of health (SDHs) are now recognized as a major factor affecting patient health outcomes and should be addressed by the health care system. Many medical students do not receive practical training on how to assess SDH needs or connect individuals with appropriate resources.

Methods: The authors developed and delivered a 90-minute case-based workshop to train undergraduate medical students in assessing SDH needs and connecting with community resources during their service-learning experiences.

An academic medical center library and community outreach office partnered with community-based organizations and public libraries to deliver patient empowerment workshops within the community. The workshops included information on reliable sources for health information and how to engage your medical provider as a partner in improving your health. The initial project expanded to include the creation of professional development workshops for area public librarians.

This study aimed to evaluate a community-based implementation of an evidence-based fall prevention program, in which 131 individuals participated in Tai Chi: Moving for Better Balance. Self-report and functional performance assessments included demographics, health and fall history, the Activities-Specific Balance Scale, the Timed Up and Go test, and the Functional Reach test. Pre-post scores were compared with the Wilcoxon signed rank test.
