Introduction: Cardiac gunshot injuries are fatal and very challenging to manage for surgeons. Unfortunately, there is no consensus regarding the management of these patients.
Case Presentation: We report an unusual case of a cardiac injury after an indirect gunshot to the heart.

- - The surgical technique involved enlarging the right ventricular outflow tract beneath the mobilized coronary artery, successfully relieving obstruction in all patients without injuring the artery.
- - The results showed no in-hospital deaths or need for additional procedures on the coronary anomaly, indicating that mobilizing the artery often eliminates the need for extra support devices like an extracardiac conduit.
