Single crystal X-ray analyses of the -methyl and -ethyl derivatives were performed and revealed that each compound adopted a spherical structure with chirality because of the fixed axis rotation and combined polarity of the amide bonds. Enantiomers of each spherical macrocycle were separated by chiral column chromatography and showed mirror-imaged CD spectra to each other.

Case Presentation: We report four cases of SMVT that required careful observation and bowel resection. Case 1: A 71-year-old man presented with abdominal pain, diarrhea, and vomiting that showed a completely occluded SMV with thrombus and small bowel ischemia.

The excellent long-term patency of no-touch (NT) saphenous vein grafts (SVGs) makes the grafts very attractive for coronary artery bypass grafting; however, NT-SVG harvesting has a greater incidence of wound complications than conventional methods. Since 2009, we have performed endoscopic vein harvesting (EVH) in our department with very few major wound complications. Because NT-SVG harvesting is expected to provide long-term patency, if performed with EVH, the incidence of wound complications will be reduced.

Methods: Using our extraction protocol, we retrospectively analyzed summary tables in the DPC database from 2016 to 2019, which were provided by the Ministry of Health, Labor and Welfare. A total of 27,278 patients were available (SAVR, n = 12,534; TAVI, n = 14,744).
We encountered a patient in a state of shock who required venoarterial extracorporeal membrane oxygenation in whom cardiac computed tomography was instrumental in diagnosing obstructive mechanical mitral valve thrombosis as well as in the differentiation of other probable diseases. Because the patient was on venoarterial extracorporeal membrane oxygenation support, computed tomography imaging required some ingenuity. ().

Methods: Forty-four patients who underwent TAR with FET for acute aortic dissection at our hospital since 2014 were included. The aortic diameter obtained from FET was measured on postoperative computed tomography (CT) and the estimated oversizing ratio was calculated.
Objectives: To evaluate the outcomes of total arch replacement using the frozen elephant trunk technique with a FROZENIX J Graft for patients with either acute or chronic aortic dissection, and to evaluate the late-phase outcomes.
Methods: Between January 2015 and December 2020, we used the frozen elephant trunk technique in 47 patients with acute aortic dissection and 12 patients with chronic aortic dissection. The primary endpoints were 30-day mortality, late aorta-related death and late aortic events.

We herein report a case of leg malperfusion caused by dynamic obstruction after aortic dissection diagnosed by the exercise ankle brachial pressure index test that could not be diagnosed solely by examining the symptoms and investigations at rest. This case suggests that exercise can be a key factor in the diagnosis of this complication. Furthermore, blood pressure elevation can be an exacerbating factor in dynamic obstruction.
