A 58-year-old male complaining of pollakisuria, miction pain and back pain visited us Dec. 26, 1979. Rectal examination revealed the prostate enlarged by 5 digital width, stony hard and irregular.

[Living related kidney transplantation after removing the aneurysm of graft: a case report].

A case of living related renal transplantation using a kidney with renal arterial aneurysm is reported. The size of the renal arterial aneurysm was 15 mm in diameter. After ex vivo aneurysm removal, the graft was transplanted to the right iliac fossa.

A C2 symmetry-based HIV protease inhibitor, A77003, irreversibly inhibits infectivity of HIV-1 in vitro.

A C2 symmetry-based HIV protease inhibitor, A77003, exerts potent antiviral activity against a wide spectrum of HIV isolates in vitro. In this study, we asked whether A77003 could cause irreversible conformational changes to HIV-1, whether the amounts of viral RNA and p24 capsid protein per virion were altered, and how the infectivity of the virus produced in the presence of the drug was affected. We found that the number of viral particles and per-virion viral RNA content of the virus produced in the presence of A77003 did not significantly differ from those of the virus produced in the absence of the drug, whereas significant morphological changes were observed as assessed by transmission electron microscopy.

[Thiazide treatment for calcium urolithiasis in patients with idiopathic hypercalciuria].

The prophylactic effect of hydrochlorothiazide on stone formation was studied in 66 patients with calcium urolithiasis, (49 men and 17 women between 25 and 76 years old, mean 52.2 +/- 11.7 years) due to idiopathic hypercalciuria.

[Primary malignant lymphoma of the urinary bladder achieving complete response by COMPA intraarterial chemotherapy: a case report].

A case of primary malignant lymphoma of the bladder is reported. A 70-year-old woman was admitted to our clinic with the chief complaint of intermittent gross hematuria on March 31, 1992. Examination of cystoscopy, IVP, ultrasonography and CT scan suggested a non-papillary invasive bladder tumor.

Protein binding of human immunodeficiency virus protease inhibitor KNI-272 and alteration of its in vitro antiretroviral activity in the presence of high concentrations of proteins.

KNI-272 represents a peptide-based protease inhibitor having potent antiretroviral activity against human immunodeficiency virus (HIV) in vitro. The structure contains allophenylnorstatine [(2S,3S)-3-amino-2-hydroxy-4-phenylbutyric acid] with a hydroxymethylcarbonyl isostere. We asked whether this experimental anti-HIV agent could exert its activity in vitro in the presence of relatively high concentrations of fetal calf serum (FCS) and assessed its protein-binding properties by using fresh human plasma preparations.

Comparison of effects of nicardipine and trichlormethiazide on insulin sensitivity in hypertensive patients.

We performed a randomized, crossover study comparing the effects of nicardipine and trichlormethiazide on insulin sensitivity in 8 untreated essential hypertensive patients. They were treated either with nicardipine or trichlormethiazide for 12 weeks, after which they were treated with the alternative drug for a further 12 weeks. Insulin sensitivity was determined by the hyperinsulinemic euglycemic clamp technique.

Effect of insulin on sodium reabsorption in hypertensive patients.

The effect of insulin on sodium reabsorption was studied with the hyperinsulinemic euglycemic clamp technique, using lithium clearance as a marker of sodium reabsorption in the proximal tubule. The subjects were 12 nondiabetic hypertensive patients who were divided into two groups. In the first group, the effect of insulin on sodium and lithium reabsorption was examined.

Insulin sensitivity test using a somatostatin analogue, octreotide (Sandostatin).

To assess the validity of the steady-state plasma glucose (SSPG) method using octreotide (Sandostatin), we compared the SSPG method with the euglycemic hyperinsulinemic clamp technique in ten non-obese, insulin-dependent diabetic patients. The SSPG method was performed by intravenous infusion of octreotide (0.5 microgram/min), insulin (2 mU/kg/min) and glucose (9 mg/kg/min) for 180 min.

Neglect in three dimensions.

Objectives: Neglect in vertical and radial directions is rarely evaluated systematically in patients with brain damage.

Method And Results: Using the line bisection task, we confirmed that some patients with unilateral brain damage manifest vertical or radial spatial neglect. In fact, patients with severe left horizontal neglect tend to have severe vertical neglect.

Laparoscopy-assisted radical nephrectomy without pneumoperitoneum.

A small renal cell carcinoma of the right kidney was completely removed from a 59-year-old women by laparoscopic radical nephrectomy without requiring a pneumoperitoneum. A 5-cm midline laparotomy incision was made and 3 small retractors were used for suspending the abdominal wall. Under laparoscopic observation, we safely positioned three trocars.

Effects of NG-monomethyl-L-arginine, indomethacin, and aspirin on the vasodepressor response to low doses of endothelin-1 and endothelin-3 in rats.

Low doses (< or = 100 pmol/kg) of endothelin-1 (ET-1) and endothelin-3 (ET-3) elicited a fall in mean arterial pressure (MAP) and a dose-related reduction in hindquarter vascular resistance in anesthetized rats. The depressor response to low doses of ET-1 or ET-3 was short-lived (about 10 sec) and reversible. High doses (> or = 200 pmol/kg) of ET-1 and ET-3 elicited a biphasic response; an initial fall in MAP followed by a gradual increase in MAP.

[Results of combination chemotherapy with etoposide, ifosfamide, peplomycin for advanced prostatic cancer].

Between August, 1986 and August, 1992, 16 combination chemotherapies with etoposide (100 mg/body, day 1-5), ifosfamide (50 mg/kg, day 1, 3, 5), peplomycin (5 mg/body, day 1-5) were performed on 13 patients with endocrine therapy-relapsed advanced prostatic cancer. Seven trials were performed on 5 patients who received DESP (diethylstilbestrol diphosphate) (500 mg/body, day 1-5) with the chemotherapy. In 9 trials performed on 9 patients who did not receive DESP, there was no response case.

A case of myelodysplastic syndrome progressing to acute myelocytic leukemia in which adult-onset Still's disease had occurred 6 years before.

A patient with adult-onset Still's disease who presented with myelodysplastic syndrome (MDS) after a course of 6 years is reported. To our knowledge, this is the first such reported case. The patient died of acute myelocytic leukemia.

Expression of MDR1 gene in acute leukemia cells: association with CD7+ acute myeloblastic leukemia/acute lymphoblastic leukemia.

MDR1 gene expression was examined in acute leukemia cells from 75 Japanese patients at diagnosis (50 with acute myeloblastic leukemia [AML]: 10 M1, 18 M2, 5 M3, 8 M4, 9 M5; 25 with acute lymphoblastic leukemia [ALL]: 13 B-precursor, 12 T-lineage). The results of MDR1 mRNA expression by reverse transcriptase polymerase chain reaction were confirmed by immunostaining using the anti-P-glycoprotein monoclonal antibody UIC2 and by a functional study using the rhodamine efflux test. Morphologically, AML M1 cases had the highest incidence of MDR1 gene expression (6 of 10 patients).

[An incidentally found asymptomatic pheochromocytoma: a case report--successful removal laparoscopic adrenalectomy].

A 31-year-old male was admitted to our hospital for further examination of a right adrenal mass, which was incidentally discovered by abdominal ultra-sonography in another hospital. The adrenaline and noradrenaline in the plasma and the adrenaline and VMA in the urine were slightly elevated, and catecholamines in the blood from the right adrenal vein were markedly increased. Glucose tolerance test showed a borderline case.

Laparoscopic adrenalectomy: clinical experience with 12 cases.

Laparoscopic adrenalectomy is a new procedure for the treatment of adrenal tumors. The clinical experience with this operation in 12 patients is reported. While adrenal tumors are relatively small and seem to be good candidates for laparoscopic surgery, this procedure is difficult because of the anatomical location.

Elevated plasma levels of vascular endothelial cell markers in patients with hypercholesterolemia.

Hypercholesterolemia is associated with an increased incidence of vascular complications. To assess the actual degree of activation of coagulation systems and vascular disorders in hypercholesterolemia, plasma levels of vascular endothelial cell markers, such as thrombomodulin (TM), tissue-type plasminogen activator, plasminogen activator inhibitor-I (PAI-I), and von Willebrand factor, were measured in 51 patients with hypercholesterolemia. We also investigated the effects of Pravastatin, an inhibitor of 3-hydroxy-3-methylglutaryl coenzyme A reductase, on plasma lipid, lipoprotein a, and hemostatic markers.

[Experience of laparoscopic pelvic lymphadenectomy in 10 patients].

From November 1991 through January 1993, we performed laparoscopic lymphadenectomy on 10 patients. These patients were aged 54 to 77 years. All laparoscopic procedures were performed under general anesthesia.
